摘 要:目的:探讨与分析血清内毒素水平对脑血管患者经择期介入手术治疗后支架内再狭窄的预测价值。方法:选择脑血管疾病患者110例作为研究对象,所有患者都给予择期介入手术治疗,检测术前1 d血清内毒素水平,随访调查患者再狭窄发生情况并进行预测价值分析。结果:在110例患者中,术后1年发生再狭窄21例(狭窄组),发生率为19.1%,其中轻度17例,中度3例,重度1例。两组术前1 d的空腹血糖、血脂水平对比差异无统计学意义(均P>0.05),狭窄组的血清内毒素水平高于非狭窄组(P<0.05)。两组术后14 d的颈动脉狭窄率都低于术前1 d(P<0.05),手术前后组内与组间对比差异都无统计学意义(均P>0.05)。在110例患者中,Spearsman分析显示术后再狭窄与内毒素存在相关性(r=0.672,P=0.000)。Logistic回归分析显示内毒素(OR=5.305,P=0.011)为导致术后再狭窄发生的主要影响因素(P<0.05)。受试者工作特征曲线(ROC)分析显示内毒素对脑血管介入术后再狭窄的预测曲线下面积为0.789。结论:脑血管介入术后再狭窄比较常见,术前血清内毒素水平对于预测术后再狭窄的发生具有重要价值。Objective:To explore and analyze the predictive value of serum endotoxin level on in stent restenosis in patients with cerebrovascular disease after elective interventional therapy.Methods:110 cases of patients with cerebrovascular disease were selected as the research objects.All patients were treated with elective interventional surgery.The serum endotoxin level was detected one day before surgery,and patients were followed up to investigate restenosis,and predictive value analysis was performed.Results:Among 110 patients,restenosis occurred in 21 cases(stenosis group)one year after operation,with an incidence of 19.1%,including 17 cases of mild,3 cases of moderate and 1 case of severe.There was no significant difference in fasting blood glucose and blood lipid levels between the two groups one day before operation(all P>0.05).The level of serum endotoxin in stenosis group was higher than that in non stenosis group(P<0.05).The carotid stenosis rate of the two groups at 14 days after operation were lower than that at 1 day before operation(all P<0.05).There was no significant difference between the two groups before and after operation(all P<0.05).Spearman analysis showed that there was a correlation between postoperative restenosis and endotoxin in 110 patients(r=0.672,P=0.000).Logistic regression analysis showed that endotoxin was the main factor leading to postoperative restenosis(OR=5.305,P=0.011).Receiver operating characteristic(ROC)curve analysis showed that the area under the curve of endotoxin predicting restenosis after cerebrovascular intervention was 0.789.Conclusion:Restenosis after cerebrovascular intervention is common.Preoperative serum endotoxin level is of great value in predicting the occurrence of postoperative restenosis.
