上睑下垂合并内眦赘皮矫正的临床观察

摘　　要：目的:探讨上睑下垂合并内眦赘皮一次性矫正的临床效果观察。方法:选取2018年3月-2020年3月笔者科室收治的先天性上睑下垂合并内眦赘皮患者107例(132只眼)进行治疗。提上睑肌肌力小于4mm67例(69只眼)采用CFS悬吊提上睑肌缩短联合“Z”成形术,提上睑肌肌力大于4mm40例(63只眼)采用提上睑肌缩短或折叠联合“Z”成形术。术后观察睑裂长度、睑裂高度、睑裂倾斜度、内眦宽度及复发率、并发症。结果:本组107例患者(132只眼)术前、术后对比,睑裂长度平均增加(2.0±1.0)mm,内眦宽度平均缩小(4.0±2.0)mm,睑裂高度平均增加(9.0±1.0)mm,睑裂倾斜度平均为(12±2)°。行CFS悬吊提上睑肌缩短术联合“Z”内眦成形术术后复发率3.0%(2/67),未复发97.0%(65/67),并发症3.0%(2/67);行提上睑肌缩短或折叠联合“Z”内眦成形术术后复发率2.5%(1/40),未复发97.5%(39/40),并发症2.5%(1/40)。结论:一次性将上睑下垂合并内眦赘皮进行矫正,改善了眼睛的内眦宽度、睑裂高度、睑裂倾斜度,整体完善了上睑的外形美,有一定的临床意义。Objective To explore the clinical effect of one-time correction of ptosis combined with internal canthal epidermis.Methods A total of 107 patients(132 eyes)with congenital blepharoptosis and epicanthus epidermis treated in the author's department from March 2018 to March 2020 were selected for treatment.The levator muscle strength was less than 4mm in 67 cases(69 eyes)with CFS suspension levator palpebral muscle shortening combined with"Z"angioplasty,and the levator eyelid muscle strength was greater than 4mm in 40 cases(63 eyes)with levator palpebral muscle shortening or folding combined"Z"plastic surgery.The length of the palpebral fissure,the height of the palpebral fissure,the inclination of the palpebral fissure,the width of the inner canthus,the recurrence rate,and the complications were observed after the operation.Results Compared with 107 patients(132 eyes)before and after surgery,the average length of palpebral fissure increased by(2.0±1.0)mm,the width of inner canthus decreased by(4.0±2.0)mm,the average height of palpebral fissure was(9.0±1.0)mm,and the average inclination of palpebral fissure was(12±2)°.After CFS suspension shortening of levator palpebrae superioris combined with"Z"canthoplasty,the recurrence rate was 3.0%(2/67),no recurrence 97.0%(65/67),and complications were 3.0%(2/67).After levator palpebrae shortening or folding combined with"Z"canthoplasty,the recurrence rate was 2.5%(1/40),no recurrence rate was 97.5%(39/40)and complications were 2.5%(1/40).Conclusion Correction of ptosis and medial epicanthus at one time improves the width of the medial canthus of the eye,the height of the eyelid fissure,and the inclination of the eyelid fissure,and the overall beauty of the upper eyelid is improved,which has certain clinical significance.
