摘　　要：目的:探讨一种治疗嵌甲性甲沟炎的改良术式。方法:选择2014年1月-2018年12月在南通大学附属医院皮肤科门诊确诊的嵌甲性甲沟炎者为研究对象,向患者详细介绍手术方法的具体情况并由患者自由选择术式,从中筛选出采用拔甲术和采用套管术的患者各29例,分别为对照组和观察组。对照组采用传统的全拔甲术或部分拔甲术,观察组采用部分甲基质切除联合套管缝合固定的方法,对比两组患者的疼痛度、创面愈合时间及治愈率。结果:观察组术后疼痛度、创面愈合时间和治愈率均优于对照组,差异有统计学意义(P<0.05);随访6个月,观察组1例患者因过度运动导致术后第20天时丝线断裂及套管脱落,重新置入套管缝扎固定后恢复良好。结论:采用部分甲基质切除联合套管缝合固定治疗嵌甲性甲沟炎,术后创面愈合快,复发率低,值得临床推广应用。Objective To introduces a modified operation for the treatment of incarcerated paronychia.Methods Patients with incarcerated Paronychia diagnosed in the dermatological outpatient department of Nantong University Hospital from January 2014 to December 2018 were selected.To introduce the specific situation of the two surgical methods to the patient in detail and allow the patient to choose the operative method freely.Twenty-nine patients with nail extraction and cannula were selected as the control group and observation group.The control group was treated with traditional total or partial nail extraction,while the observation group was treated with partial nail matrix resection combined with cannulation andfixation.The pain degree,wound healing time and cure rate of the two groups were compared.Results The postoperative pain degree,wound healing time and cure rate of the observation group were all better than those of the control group,with statistically significant differences(P<0.05).The patients were followed up for 6 months.One patient in the observation group suffered from silk thread fracture and casing fall off on the 20th day after surgery due to excessive exercise,and recovered well after cannula suturefixation.Conclusion The method of partial substrates excision combined with cannula suture andfixation in the treatment of incarcerated paronychia has the advantages of fast wound healing and low recurrence rate,which is worthy of clinical promotion.
