内镜下金属钛夹联合肾上腺素对上消化道出血疗效的影响

摘　　要：目的探讨内镜下金属钛夹联合肾上腺素对上消化道出血(非静脉曲张出血)疗效的影响。方法纳入2018年6月—2020年6月入住我院的上消化道出血(非静脉曲张出血)患者120例,根据随机数字表法分成对照组与观察组,每组均60例,对照组采用内镜下注射肾上腺素,观察组在内镜下注射肾上腺素联合金属钛夹治疗,对比两组临床疗效、住院天数、症状改善时间(引流管转清时间、呕血消失时间、大便转阴时间)、血红蛋白水平、胃肠功能评分及不良反应发生率的差异。结果对照组临床总有效率为75.00%,观察组临床总有效率较对照组明显升高,其临床总有效率为93.33%,两组对比差异有统计学意义(P=0.006)。观察组住院时间、引流管转清时间、呕血消失时间、大便转阴时间均低于对照组,两组对比差异有统计学意义(均P <0.01)。两组治疗后血红蛋白水平明显升高,胃肠功能评分明显下降,且观察组改善幅度优于对照组,差异有统计学意义(均P <0.01或P <0.05)。两组不良反应发生率比较,差异无统计学意义(P=0.488)。结论内镜下金属钛夹联合肾上腺素治疗上消化道出血疗效显著,可缩短住院天数、降低出血时间、提高止血效果、保护胃肠功能,且安全性高。Objective To investigate the effect of endoscopic metal titanium clips combined with adrenalin on upper gastrointestinal bleeding(nonvariceal hemorrhage). Methods A total of 120 cases of patients with upper gastrointestinal bleeding(non-variceal hemorrhage) were selected from June 2018 to June 2020 in our hospital, which were divided into the control group and the observation group according to random number table method, 60 cases in each group. The control group was received endoscopic adrenalin injection, but the observation group received the received endoscopic metal titanium clips combined with adrenalin injection.The clinical efficacy, hospitalization time, symptom improvement time,the levels of hemoglobin, gastrointestinal function score and the adverse reactions rate were compared between the two groups. Results The total clinical effective rate of the control group was 75.00%. The total clinical effective rate of the observation group was significantly higher than that of the control group, which rate was 93.33%(P=0.016). All the hospitalization time, drainage tube clearing time, hematemesis disappear time, stool to turn negative time in the observation group were lower than the control group(all P < 0.01). After the treatment, all the levels of hemoglobin in the two groups were significantly incerased, but the gastrointestinal function score in the two groups were significantly decerased. The descender of the observation group were better than that of the control group(all P < 0.01 or P < 0.05). There was no significant difference in total adverse reactions rate between the two groups after treatment(P=0.488). Conclusion Endoscopic metal titanium clips combined with adrenalin has a definite clinical effect for upper gastrointestinal bleeding, which can shorten the hospitalization time, reduce the bleeding time, improve hemostatic effect, protect the gastrointestinal function, and with high safety.
