更年安神方治疗阴虚火旺证患者临床疗效观察

摘　　要：目的观察更年安神方联合阿普唑仑片治疗阴虚火旺证失眠疗效。方法使用随机平行对照方法,将60例门诊患者(2018年7月—2019年12月)按随机平行对照方法分为两组,对照组30例,阿普唑仑片(0.4 mg/片),0.4 mg睡前口服,治疗组30例,更年安神方(地骨皮15 g、银柴胡10 g、合欢皮15 g、炒酸枣仁15 g、山茱萸15 g),l剂/d,水煎200 mL,早、晚饭后半小时温服。西药治疗同对照组,连续治疗4周为1疗程,观察临床症状、匹兹堡睡眠质量指数(Pittsburgh sleep quality index,PSQI)评分和神经递质相对功率、不良反应。连续治疗1个疗程,判定疗效。结果治疗组痊愈12例,显效9例,有效7例,无效2例,总有效率93.33%;对照组痊愈7例,显效6例,有效9例,无效8例,总有效率73.33%。治疗组临床疗效优于对照组(P <0.05);治疗前两组对比差异无统计学意义(P> 0.05),治疗一个疗程(4周),两组睡眠质量评分均能改善,与治疗前对比P <0.05,治疗组睡眠质量评分明显低于对照组P <0.05;治疗前两组脑内神经递质相对功率对比,P> 0.05,两组有可比性,治疗一个疗程(4周),两组均有改善,与治疗前对比P <0.05,治疗后组间对比P <0.05。结论更年安神方联合阿普唑片仑治疗阴虚火旺证失眠患者疗效满意,提高睡眠质量,且无严重不良反应,安全可靠,优于单纯西药治疗。Objective To observe the curative effect of GengNianAnShen decoction combined with alprazolam treating patients with hyperactivity of fire and yin deficiency with insomnia. Methods Using a randomized parallel control method, 60 outpatients(From July 2018 to December 2019) were divided into two groups according to the random number table method. Alprazolam tablet(0.4 mg/tablet), oral 0.4 mg before bedtime. The treatment group of 30 cases, Gengnian Anshen decoction(lycii cortex 15 g,radix stellariae10 g, cortex albizziae 15 g, parched semen ziziphi spinosae 15 g, cornus officinalis 15 g), 1 dose/d, boil to 200 mL, be taken lukewarm half an hour after breakfast and dinner. The clinical symptoms, Pittsburgh Sleep Quality Index(PSQI) score, relative power of neurotransmitters,and adverse reactions were observed. Continuous treatment for 1 course,and then determine the efficacy. Results In the treatment group, 12 cases were cured, 9 cases were effective, 7 cases were effective, 2 cases were ineffective, the total effective rate was 93.33%. In the control group, 7 cases were cured, 6 cases were effective, 9 cases were effective and 8 cases were ineffective. The total effective rate was 73.33%. The clinical effect of treatment group was better than control group(P < 0.05).Before treatment, there was no difference between the two groups(P >0.05). After a course of treatment(4 weeks), the sleep quality score of both groups was improved, compared with before treatment P < 0.05,and the sleep quality score of the treatment group was significantly lower than that of the control group(P < 0.05). Before treatment, the relative power of neurotransmitters in the brain of the two groups was compared, P > 0.05, and the two groups were comparable. After a course of treatment(4 weeks), both groups improved, P < 0.05 compared with before treatment, and P < 0.05 compared with between groups after treatment.
