2型糖尿病患者肾糖阈的性别差异及相关因素分析

摘　　要：目的分析2型糖尿病患者肾糖阈(RT_(G))的性别差异及相关因素。方法选取阜阳市人民医院内分泌科住院的114例2型糖尿病患者,分男性组(n=57)与女性组(n=57),公式法得到患者的RT_(G),比较并分析男女患者的RT_(G)、生化及临床资料。结果2型糖尿病患者平均RT_(G)值为(11.25±2.68)mmol/L;两组患者的RT_(G)值、RT_(G)分类、年龄、病程、低密度脂蛋白(LDL-C)、高密度脂蛋白(HDL-C)及血糖平均值(MBG)的差异有统计学意义(P<0.05),空腹血糖、糖化血红蛋白(HbA1c)、空腹C肽、胆固醇(CHO)、三酰甘油(TG)、血肌酐、尿酸、24 h尿糖、身体质量指数(BMI)及高血压病史差异无统计学意义(P>0.05)。线性回归分析显示,BMI(β=0.379,95%CI:0.272~0.487)、HbA1c(β=0.668,95%CI:0.471~0.866)和性别(β=1.912,95%CI:1.164~2.661)是2型糖尿病患者RT_(G)的影响因素;BMI(β=0.314,95%CI:0.156~0.437)和HbA1c(β=0.643,95%CI:0.387~0.900)是男性2型糖尿病患者RT_(G)的影响因素;BMI(β=0.425,95%CI:0.272~0.578)是女性2型糖尿病患者RT_(G)的影响因素(P<0.05)。结论性别是2型糖尿病患者RT_(G)的影响因素,女性的RT_(G)值高于男性。Objective To investigate the gender differences and related factors of renal glucose threshold(RT_(G)) in patients with type 2 diabetes.Methods A total of 114 patients with type 2 diabetes hospitalized in the Department of Endocrinology,Fuyang People’s Hospital were divided into the male groups(n=57)and the female groups(n=57).The RT_(G) of the patients was obtained by formula method.The renal glucose threshold,biochemical and clinical data of male and female patients were compared and analyzed.Results The average RT_(G) value of diabetic patients was(11.25±2.68)mmol/L.The RT_(G) value,RT_(G) classification,age,course of disease,low-density lipoprotein(LDL-C),high-density lipoprotein(HDL-C)and blood glucose mean(MBG)of the two groups of patients were different(P<0.05),while fasting blood glucose,glycosylated hemoglobin(HbA1c),fasting C-peptide,cholesterol(CHO),triglycerides(TG),blood creatinine,uric acid,24 h urine glucose,body mass index(BMI)and history of hypertension were not different(P>0.05).Linear regression analysis showed that BMI(β=0.379,95%CI:0.272~0.487),HbA1c(β=0.668,95%CI:0.471~0.866)and gender(β=1.912,95%CI:1.164~2.661)were the influencing factors of RT_(G)in patients with type 2 diabetes;BMI(β=0.314,95%CI:0.156~0.437)and HbA1c(β=0.643,95%CI:0.387~0.900)were the RT_(G)of male patients with type 2 diabetes influencing factors;BMI(β=.4258,95%CI:0.272~0.578)was influencing factors of RT_(G)in female patients with type 2 diabetes(P<0.05).Conclusion Gender is an influencing factor for the RT_(G)of patients with type 2 diabetes.The RTG value of women is higher than that of men.
