摘 要:目的通过产时超声监测产程中胎方位及脊柱位置,探讨其对持续性枕后位的预测价值。方法选取单胎阴道分娩产妇106例作为研究对象,产程中应用腹部及会阴超声监测胎方位及胎儿脊柱位置,同时记录胎儿分娩时的胎方位,分析超声评估胎方位及脊柱位置对持续性枕后位的预测价值。结果第二产程中超声评估胎方位对持续性枕后位分娩预测的灵敏度为100%,特异度为77%,阳性预测值为31%;第二产程中超声评估胎儿脊柱位置预测枕后位分娩的灵敏度为100%,特异度为96%,阳性预测值为71%。结论第二产程超声评估胎方位及胎儿脊柱位置为较好的持续性枕后位的预测方式,且胎儿脊柱位置的预测价值高于胎方位。Objective To investigate the value of ultrasound monitoring of fetal position and spine position during the first and second stages of labor in predicting delivery in persistent occipitoposterior position.Methods A total of 106 pregnant women with vaginal delivery of singleton fetus were enrolled as subjects.Transabdominal and perineal ultrasound was performed during labor to monitor fetal position and spine position,and the position of the fetus was recorded at the time of birth.The value of ultrasound monitoring of fetal position and spine position in predicting delivery in persistent occipitoposterior position was analyzed.Results Ultrasound monitoring of fetal position in the second stage of labor had a sensitivity of 100%,a specificity of 77%,and a positive predictive value of 31%in predicting delivery in persistent occipitoposterior position.Ultrasound monitoring of fetal spine position in the second stage of labor had a sensitivity of 100%,a specificity of 96%,and a positive predictive value of 71%in predicting delivery in persistent occipitoposterior position.Conclusion Ultrasound monitoring of fetal position and fetal spine position in the second stage of labor can help to predict delivery in persistent occipitoposterior position,and fetal spine position has a higher predictive value than fetal position.
