补阳还五汤加减治疗痰瘀痹阻型类风湿关节炎的临床应用效果

摘　　要：目的:分析补阳还五汤加减治疗痰瘀痹阻型类风湿关节炎的临床应用效果。方法:选取我院2017年1月—2019年12月收治的痰瘀痹阻型类风湿关节炎患者120例,采用随机数字表法分为对照组和治疗组,每组60例。对照组给予甲氨蝶呤片以及塞来昔布治疗,治疗组在对照组基础上联合补阳还五汤加减治疗,比较两组临床疗效、血沉水平(ESR)以及C反应蛋白(CRP)、中医症候积分及不良反应发生情况。结果:治疗组总有效率高于对照组,差异有统计学意义(P<0.05);治疗组ESR、CRP水平低于对照组,差异有统计学意义(P<0.05);治疗组关节疼痛、关节肿胀、晨僵积分低于对照组,差异有统计学意义(P<0.05);两组不良反应发生率比较,差异无统计学意义(P>0.05)。结论:补阳还五汤加减治疗对痰瘀痹阻型类风湿关节炎疗效确切,可改善患者临床症状,降低炎性指标,且联合用药不增加不良反应发生风险,应用安全有效。Objective:To analyze the clinical application effect of Buyanghuanwu decoction in the treatment of rheumatoid arthritis of phlegm-stasis obstruction type.Methods:A total of 120 patients with rheumatoid arthritis of phlegm and blood stasis type admitted to our hospital from January 2017 to December 2019 were selected and randomly divided into control group and treatment group,with 60 cases in each group.The control group was treated with methotrexate tablets and celecoxib,and the treatment group was treated with Buyanghuanwu decoction on the basis of the control group.The clinical efficacy,erythrocyte sedimentation rate(ESR),C-reactive protein(CRP),TCM symptom score and adverse reactions were compared between the two groups.Results:The total effective rate of the treatment group was higher than that of the control group,and the difference was statistically significant(P<0.05).The levels of ESR and CRP in the treatment group were lower than those in the control group,and the difference was statistically significant(P<0.05).The scores of joint pain,joint swelling and morning stiffness in the treatment group were lower than those in the control group,and the difference was statistically significant(P<0.05).There was no significant difference in the incidence of adverse reactions between the two groups(P>0.05).Conclusion Buyanghuanwu decoction is effective in treating rheumatoid arthritis of phlegm-stasis obstruction type,which can improve the clinical symptoms of patients,reduce inflammatory indexes,and the combination of drugs does not increase the risk of adverse reactions,so it is safe and effective.
