从易水学派调补脾胃学术思想探析咳喘辨治经验

摘　　要：易水学派精于脏腑辨证论治,在先后天元气论的认识基础上,重视后天元气之本在治疗咳喘中的重要性,同时强调脾胃为人体气机升降枢纽,脾胃受损是导致肺失宣降的关键病因之一。易水学派将脾胃学说运用于咳喘的临床辨治中,以补脾胃、调气机、培土生金为其治疗咳喘的治疗思路,灵活运用甘温、升浮之药。通过梳理易水学派对咳喘病因病机的认识及辨治咳喘的治则治法经验,分析其代表医案,进一步挖掘易水学派调补脾胃辨治咳喘的学术观点及组方遣药规律,为临床辨治咳喘提供更多思路及参考依据。Yishui School was extremely proficient in dialectical treatment of viscera.Based on the understanding of sequence vitality theory,acquired vitality was of primary importance on cough with asthma.Meanwhile,Yishui School highlighted that the spleen and stomach was hub of ascending and descending of functional activities of vitality,impaired spleen and stomach was one of key causes for dyspnea.Spleen and Stomach Theory put into practice,Chinese herbs of Ganwen and Shengfu were flexibly used according to the treatment thoughts of tonifying spleen and stomach,regulating qi,supplementing spleen to nourish lung.This article summarizes Yishui School's understanding and treatment experience of the etiology,pathogenesis,syndrome differentiation of cough with asthma,and analyzes its representative medical records,aiming to dig out the academic viewpoints regareding Yishui School's cough with asthma,and master the law of medication,in order to provide more ideas and reference for clinical treatment of cough with asthma in further.
