釜底抽薪方贴敷涌泉穴治疗肝阳上亢型原发性高血压的临床研究

摘　　要：目的观察釜底抽薪方贴敷涌泉穴治疗肝阳上亢型原发性高血压的临床疗效。方法将60例肝阳上亢型原发性高血压患者按照随机数字表法分为2组,对照组30例予缬沙坦氨氯地平片(Ⅰ)治疗,治疗组30例在对照组的基础上加用釜底抽薪方贴敷涌泉穴治疗。2组均治疗7 d后统计中医证候疗效和血压疗效,比较2组治疗前后中医证候[包括眩晕、耳鸣、头目胀痛、急躁易怒、颜面潮红、腰膝酸软、手足心热、舌脉]评分变化情况,比较2组晨起血压[包括收缩压(SBP)、舒张压(DBP)]、24 h动态血压[包括24 h平均收缩压(24 h MSBP)、24 h平均舒张压(24 h MDBP)]变化。结果治疗组中医证候疗效总有效率83.33%(25/30),血压疗效总有效率93.33%(28/30),对照组分别为50.00%(15/30)、73.33%(22/30),治疗组中医证候疗效和血压疗效总有效率均高于对照组(P <0.05)。治疗组治疗后中医证候眩晕、耳鸣、头目胀痛、急躁易怒、颜面潮红、腰膝酸软、手足心热、舌脉评分和总分与本组治疗前比较均降低(P <0.05),对照组治疗后仅眩晕、耳鸣、头目胀痛、急躁易怒、腰膝酸软、舌脉评分和总分与本组治疗前比较均降低(P <0.05),治疗组治疗后眩晕、头目胀痛、急躁易怒、手足心热、舌脉评分和总分均低于对照组(P <0.05)。2组治疗后SBP、DBP水平与本组治疗前比较均降低(P <0.05),且治疗组治疗后SBP、DBP水平均低于对照组(P <0.05)。2组治疗后24 h MSBP、24 h MDBP水平与本组治疗前比较均降低(P <0.05),且治疗组治疗后24 h MSBP水平低于对照组(P <0.05)。结论釜底抽薪方贴敷涌泉穴治疗肝阳上亢型原发性高血压临床疗效确切,可明显改善患者中医证候表现,降低血压水平,稳定血压变化。Objective To observe the clinical efficacy of acupoint application of Fudi Chouxin Prescription at yongquan(KI1)for essential hypertension(EH)with hyperactivity of liver yang.Methods Totally 60 EH patients(hyperactivity of liver yang)were randomly divided into equal group.On the basis of valsartan amlodipine tablet,and treatment group additionally received acupoint application of Fudi Chouxin Prescription at yongquan(KI1).Treated for 7 days,the aim was to compare efficacy of traditional Chinese medicine(TCM)syndrome and blood pressure,TCM syndrome(vertigo,tinnitus,head pain,irritability,facial flushing,lumbar debility,feverish palms and soles,vessels of tongue)scores,morning blood pressure[systolic blood pressure(SBP),diastolic blood pressure(DBP)],24 h ambulatory blood pressure(ABP)[24 h mean systolic blood pressure(24 hMSBP),24 h mean diastolic blood pressure(24 hMDBP)].Results The total effective rates of TCM syndrome and blood pressure were better in treatment group than in control group[83.33%(25/30)vs 50%(15/30),93.33%(28/30)vs 73.33%(22/30),(P<0.05),respectively].After treatment,TCM syndrome(vertigo,tinnitus,head pain,irritability,facial flushing,lumbar debility,feverish palms and soles,vessels of tongue)scores in treatment group were decreased(P<0.05);TCM syndrome(vertigo,tinnitus,head pain,irritability,lumbar debility,vessels of tongue)scores in control group were decreased(P<0.05);TCM syndrome(vertigo,head pain,irritability,feverish palms and soles,vessels of tongue)scores were decreased in treatment group when compared with cortrol group(P<0.05);SBP and DBP in groups were decreased(P<0.05),treatment group was lower than control group(P<0.05);24 h MSBP and 24 h MDBP in groups were lower(P<0.05),and 24 hMSBP in treatment group was lower as compared with control group(P<0.05).Conclusion For EH patients(hyperactivity of liver yang),the acupoint application of Fudi Chouxin Prescription at yongquan(KI1)has definite clinical curative effect,is capable of improving TCM syndrome,reducing blood pressure level,an
