手法整复加南药鸡矢藤汤对陈旧性下尺桡关节脱位患者血清炎症相关因子和肌酸激酶的影响

摘　　要：目的观察手法整复加南药鸡矢藤汤对陈旧性下尺桡关节脱位患者血清炎症相关因子可溶性细胞间黏附分子-1(sICAM-1)、C反应蛋白(CRP)和白细胞介素6(IL-6)及肌酸激酶(CK)含量的影响。方法将78例陈旧性下尺桡关节脱位患者按照随机数字表法分为2组。对照组39例予手法整复,治疗组39例在对照组基础上加鸡矢藤汤内服外敷。比较2组治疗前后疼痛视觉模拟评分(VAS)、Cooney腕关节评分、Gartland-Werley腕关节功能评分及血清sICAM-1、CRP IL-6、CK含量变化。结果治疗后,2组疼痛VSA,Gartland-Werley腕关节残余畸形、主观评价、客观评价、并发症评分及总分,血清sICAM-1、CRP IL-6、CK水平均较本组治疗前降低,且治疗组均低于对照组,比较差异均有统计学意义(P<0.05)。治疗后2组Cooney腕关节疼痛、功能状况、活动范围、握力评分及总分均较本组治疗前提高,且治疗组均高于对照组,比较差异均有统计学意义(P<0.05)。结论手法整复加鸡矢藤汤治疗陈旧性下尺桡关节脱位患者可抑制炎性反应,减轻骨骼肌损伤,促进腕关节功能恢复。Objective To observe the effect of manual reduction combined with Nanyao(Jishiteng Decoction)for obsolete dislocation of distal radioulnar joint(DRUJ)in patients and its influence on serum soluble intercellular adhesion molecules-1(sICAM-1),C reactive protein(CRP),interleukin-6(IL-6)and creatine kinase(CK).Methods Totally 78 patients with obsolete dislocation of distal radioulnar joint(DRUJ)were randomly divided into equal group,treatment group was treated with manual reduction combined with Nanyao(Jishiteng Decoction,oral medication and external application),control group with manual reduction alone.The key observation was Visual Analogue Score(VAS),Cooney wrist score,Gartland-Werley wrist joint function score,serum sICAM-1,CRP,IL-6,CK between groups before and after treatment.Results After treatment,VSA,Gartland&Werley scores(residual malformation,subjective evaluation,objective evaluation,complications),serum sICAM-1,CRP,IL-6,CK in groups were decreased,treatment group was lower than control group,and with statistically significant difference(P<0.05);Cooney wrist scores(pain,function status,movement range,grip strength)in groups were increased,treatment group was increased in comparison with control group,the difference of two groups being significant(P<0.05).Conclusion For patients with obsolete DRUJ dislocation,manual reduction combined with Jishiteng Decoction are capable of inhibiting inflammatory reaction,reducing skeletal muscle injury,and promoting the recovery of wrist joint function.
