顺气通腑汤灌肠在癌痛患者药源性便秘致阴液亏虚中的应用效果

摘　　要：目的探讨顺气通腑汤灌汤治疗癌痛患者药源性便秘致阴液亏虚的临床应用价值。方法选择2019年1月至12月赣州市肿瘤医院确诊的阴液亏虚便秘癌痛80例患者,采用随机数字表法分为试验组与对照组,每组各40例。对照组采用口服乳果糖治疗,试验组采用口服乳果糖治疗基础上联合顺气通腑汤灌肠治疗。观察两组治疗2个疗程后的近期疗效、满意度,观察两组的治疗前后便秘症状严重度量表(Wexner)及生活质量量表(PAC-QOL)评分,记录不良反应总发生率。结果试验组治疗2个疗程后的Wexner评分低于对照组,试验组治疗2个疗程后的PAC-QOL各维度评分及总分低于对照组,差异均有统计学意义(P<0.05)。试验组的临床总有效率高于对照组,差异有统计学意义(P<0.05)。两组治疗过程未出现不良反应。试验组的总满意度高于对照组,差异有统计学意义(P<0.05)。结论药源性便秘致阴液亏虚的癌痛患者经顺气通腑汤灌肠治疗的疗效较好,可改善其生活质量,提高治疗满意度,确保癌痛治疗的顺利进行。Objective To explore the clinical application value of Shunqi Tongfu Decoction enema in the treatment of Yin deficiency caused by drug-induced constipation in patients with cancer pain.Methods A total of 80 patients with Yin deficiency,constipation and cancer pain were diagnosed in Ganzhou Cancer Hospital from January to December 2019 were selected.They were divided into the control group and the experimental group according to random number table method,40 cases in each group.The control group was treated with oral Lactulose,and the experimental group was treated with oral Lactulose combined with Shunqi Tongfu Decoction enema.The short-term efficacy and satisfaction of the two groups at 2 courses after treatment were observed.The scores of constipation symptom severity scale(Wexner)and quality of life scale(PAC-QOL)in the two groups were observed before and after treatment,and the total incidence rate of adverse reactions was recorded.Results The score of Wexner scale in the experimental group at 2 courses after treatment was lower than that in the control group,the scores of each dimension of PAC-QOL in the experimental group at 2 courses after treatment were lower than those in the control group,and the differences were statistically significant(P<0.05).The clinical total effective rate of the experimental group was higher than that of the control group,and the difference was statistically significant(P<0.05).There were no adverse reactions in the two groups.The total satisfaction of the experimental group was better than that of the control group,the difference was statistically significant(P<0.05).Conclusion The Yin deficiency caused by drug-induced constipation in patients with cancer pain were treated by Shunqi Tongfu Decoction enema,and the curative effect was good,which could improve their quality of life,improve treatment satisfaction,and ensure the smooth progress of cancer pain treatment.
