摘  要:目的:运用CBCT分析下颌骨外形与下颌智齿阻生之间的关系,以及下颌骨外形各测量指标对下颌智齿是否阻生及阻生位置的影响,下颌智齿阻生的独立因素。方法:选取年龄为20~35岁之间的80名(112颗)下颌智齿阻生患者(平均年龄28.6岁)为研究对象,依据CBCT影像学检查结果,分为萌出组(56颗)和阻生组(56颗);将阻生组分为3个亚组:Ⅰ类(15颗)、Ⅱ类(24颗)、Ⅲ类(17颗)。CBCT测量分析相关9项测量指标,并进行Logistic回归分析。结果:阻生组的下颌支上端曲度、下颌支相对高度大于萌出组,下颌体部下缘长度、下后牙槽高度小于萌出组,差异有统计学意义(P<0.05),为下颌智齿阻生的保护因素。阻生组亚组组间分析表明不同阻生位置的下颌智齿的下颌骨容积的最大距离、下颌体部下缘,差异有统计学意义(P<0.05)。结论:下颌骨外形影响下颌智齿阻生与否及阻生位置,下颌体部下缘长度和下后牙槽高度为预测下颌智齿能否萌出的指标。Objective:To analyze the relationship between mandibular shape and impacted mandibular wisdom tooth by CBCT,as well as the influence of each measurement index of mandibular shape on impacted mandibular wisdom tooth and impacted position,and the independent factors of impacted mandibular wisdom tooth.Methods:Eighty patients(112 pieces)with impacted mandibular wisdom teeth aged 20~35 years(mean age 28.6 years)were selected as the research subjects.According to the results of CBCT,they were divided into eruption group(56 pieces)and impacted group(56 pieces),impacted group were divided into 3 subgroups including groupⅠ(15 pieces),groupⅡ(24 pieces)and groupⅢ(17 pieces).Totally nine measurement indicators related to CBCT measurement and analysis,and the measurement indicators were subjected to Logistic regression analysis.Results:The upper curvature of the mandibular branch and the relative height of the mandibular branch in the impacted group were greater than the eruption group,and the lower edge length of the mandibular body and the height of the lower posterior alveolar were smaller than the eruption group.The difference was statistically significant(P<0.05),which was the protective factor for impacted mandibular wisdom teeth.The analysis between subgroups of the impacted group showed that the maximum distance of mandible volume and the lower edge of mandible body of mandibular wisdom teeth at different impacted positions were statistically significant(P<0.05).Conclusion:The shape of mandible affects the location of impacted mandibular wisdom teeth.The length of the lower edge of the mandible and the height of the posterior alveolar can be used to predict the eruption of mandibular wisdom teeth.
