补肾养血填精法配合针刺治疗薄型子宫内膜不孕症临床观察

摘　　要：目的观察补肾养血填精法配合针刺对薄型子宫内膜不孕症的临床疗效。方法将南昌市洪都中医院2019年1月—2021年1月收治的120例薄型子宫内膜不孕症患者纳入研究,随机分为试验组与对照组,各60例。试验组予补肾养血填精法自拟中药方配合针刺,加入戊酸雌二醇片口服治疗;对照组单纯给予戊酸雌二醇片口服治疗。观察比较2组卵泡期的卵巢基质动脉血流变化搏动指数(Pulsatility index,PI)、阻力指数(Resistive index,RI)、子宫内膜增长情况、子宫内膜三线征阳性及宫内妊娠情况。结果试验组卵泡期的PI、RI均低于对照组(P<0.05),试验组患者的内膜厚度、宫内妊娠率及三线征阳性率均比对照组高(P<0.05),试验组患者的总有效率90.00%(54/60)明显高于对照组80.00%(48/60),差异有统计学意义(P<0.05)。结论补肾养血填精法配合针刺治疗薄型子宫内膜不孕症,能够改善卵泡期卵巢基质动脉血流变化,促进子宫内膜增长,提高子宫内膜三线征阳性率,提高受孕的几率,值得大力推广运用。Objective To observe the clinical effect of tonifying kidney, nourishing blood and filling essence combined with acupuncture on thin endometrial infertility. Methods 120 patients with thin endometriosis admitted from January 2019 to January 2021 were included in the study and randomly divided into experimental group and control group, 60 cases each. The experimental group was given oral treatment with estradiol valerate tablets, while the control group was given oral treatment with estradiol valerate tablets. The pulsation index(PI), resistance index(RI), endometrial growth, endometrium positive sign and intrauterine pregnancy were observed and compared between the two groups at follicular stage. Results In the follicular stage, PI and RI of the experimental group were lower than those of the control group(P<0.05), the endometrial thickness, intrauterine pregnancy rate and the positive rate of third-line signs of the experimental group were all higher than those of the control group( P<0.05). The total effective rate of experimental group was 90.00%(54/60) significantly higher than that of control group 80.00%(48/60), and the difference was statistically significant( P <0.05). Conclusion The treatment of thin endometrium infertility with tonifying the kidney, nourishing the blood and filling the essence combined with acupuncture can improve the changes of blood flow in the nest stromal artery, promote the growth of endometrium,improve the positive rate of third-line endometrium, and increase the probability of conception, which is worth promoting.
