摘 要:目的探讨顺行指动脉岛状皮瓣对指端缺损的修复效果。方法采用前瞻性随机试验方法,选取山东大学齐鲁医院2018年1月至2020年1月收治的40例指端缺损患者作为研究对象,采用随机数字表法将其分为A组和B组,每组20例。A组采用逆行指动脉岛状皮瓣治疗,B组采用顺行指动脉岛状皮瓣治疗,治疗8周后,比较两组皮瓣感觉功能、手术效果,并比较治疗期间两组皮瓣存活率及并发症。结果治疗8周后,B组优良率高于A组,差异有统计学意义(P<0.05)。两组近端、远端指间关节活动度比较,差异未见统计学意义(P>0.05)。B组手术时间低于A组,差异有统计学意义(P<0.05)。两组皮瓣存活率及并发症发生率比较,差异未见统计学意义(P>0.05)。结论顺行指动脉岛状皮瓣对指端缺损的修复效果较好,可有效缩短手术时间,改善皮瓣感觉功能。Objective To investigate the effect of anterograde digital artery island flap on repairing of fingertip defect.Methods A prospective randomized trial was conducted.Forty patients with fingertip defect who were admitted to Qilu Hospital of Shandong University from January 2018 to January 2020 were selected as the research objects.They were divided into group A and group B by random number table method,with 20 patients in each group.Group A was treated with retrograde digital artery island flap,and group B was treated with anterograde digital artery island flap.After 8 weeks of treatment,the sensory function and surgical effect of the two groups were compared,and the survival rate and complications of the two groups during the treatment were compared.Results After 8 weeks of treatment,the rate of excellent and good in group B was higher than that in group A,and the difference was significant(P<0.05).There was no significant difference in the proximal and distal range of motion between the two groups(P>0.05).The operative time of group B was lower than that of group A,and the difference was significant(P<0.05).There was no significant difference in the survival rate and incidence of complications between the two groups(P>0.05).Conclusions The anterograde digital artery island flap can effectively shorten the operation time and improve the sensory function of the finger tip defect.
