摘　　要：目的:探究聚乙二醇α-2a干扰素(pegIFNα-2a)与恩替卡韦联合治疗对慢性乙型肝炎(CHB)患者指标转阴情况、外周血T细胞亚群、辅助性T细胞17(Th17)细胞等水平的影响。方法:选取2018年1月-2019年1月中国航天科工集团七三一医院收治的60例CHB患者作为研究对象,按照随机数字表法分为两组,观察组和对照组各30例,对照组接受恩替卡韦治疗,观察组在对照组基础上联合pegIFNα-2a治疗,比较两组指标转阴情况、外周血T细胞亚群水平、Th17细胞水平及血生化指标水平。结果:治疗1年后,观察组乙肝病毒脱氧核糖核酸(HBV-DNA)、乙型肝炎表面抗原(HBsAg)、乙型肝炎e抗原(HBeAg)转阴率,以及CD3^(+)、CD4^(+)水平均高于对照组(P<0.05),CD8^(+)水平、Th17细胞、IL-17、ALT和AST水平均低于对照组(P<0.05)。结论:pegIFNα-2a与恩替卡韦联合应用于CHB的治疗,能提高患者HBV-DNA、HBsAg、HBeAg转阴率,调节外周血T细胞亚群和Th17细胞水平,改善血生化指标水平。Objective:To explore the effect of pegIFNα-2a combined with Entecavir on negative conversion,T cell subsets and helper T cell 17(Th17)in peripheral blood of patients with chronic hepatitis B(CHB).Method:A total of 60 patients with CHB admitted to 731 Hospital of Aerospace Science and Industry Group from January 2018 to January 2019 were selected as the research objects,and divided into two groups according to the random number table method.The patients in the control group received Entecavir,while the patients in the observation group were treated with pegIFNα-2a on the basis of the control group.The negative conversion,peripheral blood T cell subsets,Th17 cells and blood biochemical indexes of the two groups were compared.Result:After one year of treatment,the negative rate of HBV-DNA,HBsAg and HBeAg,the levels of CD3^(+)and CD4^(+)in the observation group were higher than those in the control group(P<0.05),while the levels of CD8^(+),Th17 cells,IL-17,ALT and AST were lower than those in the control group(P<0.05).Conclusion:pegIFNα-2a combined with Entecavir can improve the negative rate of HBV-DNA,HBsAg and HBeAg in patients with CHB,regulate the levels of T cell subsets and Th17 cells in peripheral blood,and improve the level of blood biochemical indicators.
