摘　　要：目的探讨检测脂蛋白相关性磷脂酶A(Lp-PLA2)在合并牙周炎的冠状动脉粥样硬化性心脏病(冠心病)患者中的临床价值。方法入选110例患者行冠状动脉(冠脉)造影明确是否冠心病,同时行牙周检查明确是否牙周炎,分为4组:对照组、牙周炎组、冠心病组、冠心病合并牙周炎组。采用ELISA法集中检测血浆Lp-PLA2浓度,分析其组间差异,以及其与Gensini积分的相关性。结果Lp-PLA2浓度:冠心病合并牙周炎组(132.31±58.23 ng/ml)明显高于对照组(98.00±41.26 ng/ml)以及牙周炎组(98.45±51.74 ng/ml),差异具有统计学意义(P=0.021,P=0.023);入选患者的Lp-PLA2水平与Gensini积分具有中度相关性(r=0.548,P<0.01)。结论Lp-PLA2水平既反映合并牙周炎的冠心病患者的冠脉病变的严重程度,也可评估合并牙周炎的冠心病患者发生心血管事件的风险。Objective To investigate the clinical value of lipoprotein-associated phospholipase A(Lp-PLA2)in patients with coronary heart disease(CHD)complicated with periodontitis.Methods One hundred and ten patients who underwent coronary angiography for definite CHD and also underwent periodontal examination for definite CHD or not were divided into four groups:control,periodontitis,CHD,and CHD combined with periodontitis.The plasma Lp-PLA2 concentration was centrally detected by the ELISA method to analyze its intergroup difference,as well as its correlation with the Gensini score.Results Lp-PLA2 concentration:CHD combined with periodontitis group(132.31±58.23 ng/ml)was significantly higher than control group(98.00±41.26 ng/ml)as well as periodontitis group(98.45±51.74 ng/ml),the difference was statistically significant(P=0.021,P=0.023).The Lp-PLA2 levels of the enrolled patients had a moderate correlation with the Gensini score(r=0.548,P<0.01).Conclusion Lp-PLA2 levels both reflect the severity of coronary lesions in CHD patients with concomitant periodontitis and assess the risk of cardiovascular events in CHD patients with concomitant periodontitis.
