摘 要:目的探讨牛奶蛋白过敏症(CMPA)患儿血清IgE及淋巴细胞亚群的测定及意义。方法选取2018年5月至2020年5月东莞市人民医院诊治的64例CMPA患儿和38例健康体检儿童的临床资料进行分析,其中CMPA患儿设立为过敏症组,健康体检儿童设立为对照组,均采用免疫比浊法与流式细胞仪检测免疫球蛋白及淋巴细胞亚群。结果过敏症组的IgE、IgG高于对照组,差异有统计学意义(P<0.05);两组IgA、IgM、C3、C4比较,差异无统计学意义(P>0.05)。过敏症组的CD_(3)^(+)、CD_(3)^(+)CD_(8)^(+)低于对照组,CD_(3)^(+)/CD_(19)^(+)、CD_(19)^(+)CD_(23)^(+)、CD_(19)^(+)高于对照组,差异均有统计学意义(P<0.05)。Pearson相关分析显示,CD_(3)^(+)、CD_(3)^(+)CD_(8)^(+)与IgE呈负相关(r=-0.303/-0.389,P<0.001),CD_(3)^(+)/CD_(19)^(+)、CD_(19)^(+)、CD_(19)^(+)CD_(23)^(+)与IgE呈正相关(r=0.588/0.785/0.759,P<0.001)。ROC曲线分析显示,IgE、CD_(3)^(+)、CD_(3)^(+)CD_(8)^(+)、CD_(3)^(+)/CD_(19)^(+)、CD_(19)^(+)CD_(23)^(+)、CD_(19)^(+)诊断CMPA的AUC值分别为0.997、0.741、0.752、0.805、0.998、0.960,(P<0.05)。结论血清IgE在CMPA患儿中呈高表达,其水平的升高会影响淋巴细胞亚群的比例及功能。Objective To investigate the determination and significance of serum IgE and lymphocyte subsets in children with milk protein allergy(CMPA).Methods The clinical data of 64 children with CMPA and 38 children with healthy physical examination who were diagnosed and treated in Dongguan People’s Hospital from May 2018 to May 2020 were selected for analysis.The children with CMPA were set up as the allergy group and the children with healthy physical examination were set up as the control group.The immunoturbidimetry and flow cytometry were used to detect immunoglobulin and lymphocyte subsets.Results The IgE and IgG in the allergy group were higher than those in the control group,and the difference was statistically significant(P<0.05).The comparison of IgA,IgM,C3,and C4 between the two groups was not statistically significant(P>0.05).CD_(3)^(+) and CD_(3)^(+)CD_(8)^(+) in the allergy group were lower than those in the control group,while CD_(3)^(+)/CD_(19)^(+),CD_(19)^(+)CD_(23)^(+),and CD_(19)^(+) were higher than those in the control group.The difference was statistically significant(P<0.05).Pearson correlation analysis showed that CD_(3)^(+),CD_(3)^(+)CD_(8)^(+) and IgE were negatively correlated(r=-303/-0.389,P<0.001),and CD_(3)^(+)/CD_(19)^(+),CD_(19)^(+),CD_(19)^(+)CD_(23)^(+)and IgE were positively correlated(r=0.588/0.785/0.759,P<0.001).ROC curve analysis showed that the AUC values of IgE,CD_(3)^(+),CD_(3)^(+)CD_(8)^(+),CD_(3)^(+)/CD_(19)^(+),CD_(19)^(+)CD_(23)^(+),CD_(19)^(+) to diagnose CMPA were(0.997,0.741,0.752,0.805,0.998,0.960,P <0.05).Conclusion Serum IgE is highly expressed in children with CMP A,and the increase in its level will affect the proportion and function of lymphocyte subsets.
