无创机械通气对伴有意识障碍的重症呼吸衰竭患者治疗价值

摘　　要：目的探讨无创机械通气治疗伴有意识障碍的重症呼吸衰竭患者的效果。方法挑选100例慢性阻塞性肺疾病合并意识障碍的重症呼吸衰竭患者作为研究对象,采用随机数字表法将所有患者分成观察组与对照组,每组50例。对照组患者应用有创通气治疗,观察组患者实施无创通气治疗,分析对比两组治疗效果。结果观察组总有效率为88.00%,对照组总有效率为68.00%,观察组总有效率高于对照组,差异具有统计学意义(P<0.05)。两组治疗后,观察组的PaO_(2)、pH值高于对照组,而PaCO_(2)、HR低于对照组,差异具有统计学意义(P<0.05)。结论无创机械通气治疗慢性阻塞性肺疾病合并意识障碍重型呼吸衰竭患者,疗效确切,可以较好改善患者动脉血气指标值,改善患者意识状态,安全可靠,值得推广应用。Objective To explore the effect of non-invasive mechanical ventilation on patients with severe respiratory failure accompanied by conscious disturbance.Methods One hundred patients with severe respiratory failure complicated with chronic obstructive pulmonary disease and consciousness disturbance were selected as research objects,and all patients were divided into observation group and control group by random number table method,with 50 cases in each group.Patients in the control group were treated with invasive ventilation,while patients in the observation group were treated with non-invasive ventilation.The therapeutic effects of the two groups were analyzed and compared.Results The total effective rate of the observation group was 88.00%,and that of the control group was 68.00%.The total effective rate of the observation group was higher than that of the control group,and the difference was statistically significant(P<0.05).After treatment of the two groups,the PaO_(2) and pH values of the observation group were higher than those of the control group,while PaCO_(2) and HR were lower than those of the control group.the difference was statistically significant(P<0.05).Conclusion Non-invasive mechanical ventilation is effective in the treatment of patients with chronic obstructive pulmonary disease complicated with severe respiratory failure due to consciousness disorder.It can better change the arterial blood gas index value of patients,improve the consciousness state of patients,is safe and reliable,and is worthy of popularization and application.
