整体护理在急性胆囊炎患者腹腔镜胆囊手术围手术期的应用效果

摘　　要：目的研究整体护理在急性胆囊炎患者腹腔镜胆囊手术围手术期的应用效果。方法选取2018年7月至2019年7月于本院进行腹腔镜胆囊手术的急性胆囊炎患者120例,经等量电脑随机法分组,两组各60例,对照组围手术期应用常规护理,包括术前各项检查、术后定期更换切口敷料,按照医嘱给药,询问详细情况等。干预组围手术期应用整体护理,主要包括心理疏导、呼吸道护理、饮食护理、活动护理等。比较两组患者术后排气时间、首次下床活动时间、住院时间、并发症发生率及护理满意度。结果干预组患者术后排气时间、首次下床活动时间、住院时间分别为(27.10±6.73)h、(16.57±6.02)h、(3.53±0.96)d,均明显短于对照组(P<0.05),并发症发生率为1.67%,明显低于对照组(11.67%),P<0.05;护理满意度为96.67%,明显高于对照组(86.67%),差异有统计学意义(P<0.05)。结论整体护理在急性胆囊炎患者腹腔镜胆囊手术围手术期具有显著的应用效果,可积极促进术后康复,及时防治术后尿潴留、胆漏、感染等并发症,能有效提高围手术期护理质量。Objective To study the effect of holistic nursing in patients with acute cholecystitis during perioperative laparoscopic cholecystectomy.Methods A total of 120 patients with acute cholecystitis who underwent laparoscopic cholecystitis surgery in our hospital from July 2018 to July 2019 were selected and divided into two groups by equal quantity computer random method,60 patients in each group.The control group received routine perioperative care,including preoperative examination,regular postoperative incision dressing replacement,medication administration according to the doctor's advice,and detailed information,etc.The intervention group used holistic nursing during perioperative period,including psychological counseling,respiratory nursing,diet nursing,activity nursing,etc.Postoperative exhaust time,first time out of bed activity,length of hospital stay,incidence of complications and nursing satisfaction were compared between the two groups.Results The postoperative exhaust time,first ambulation time and hospital stay in the intervention group were(27.10±6.73)hours,(16.57±6.02)hours and(3.53±0.96)days,respectively,which were significantly shorter than those in the control group(P<0.05),the complication rate in the intervention group was 1.67%,which was significantly lower than that in the control group(11.67%),P<0.05.Nursing satisfaction in the intervention group was 96.67%,significantly higher than the control group(86.67%),the differences were statistically significant(P<0.05).Conclusion Holistic nursing in patients with acute cholecystitis laparoscopic cholecystectomy perioperative period has significant application effect,can actively promote postoperative rehabilitation,timely prevention and treatment of postoperative urinary retention,bile leakage,infection and other complications,can effectively improve perioperative nursing quality.
