山东省农村居民家庭医生预约转诊个性化签约服务支付意愿影响因素研究

摘　　要：目的:以安德森理论为框架对山东省农村地区居民家庭医生预约转诊个性化签约服务支付意愿及影响因素进行研究,为促进我国家庭医生签约服务发展提供参考。方法:采用多阶段分层随机抽样法,对山东省2979名农村居民进行问卷调查。结果:共回收有效问卷2970份,44.8%的农村居民愿意支付家庭医生预约转诊个性化签约服务;对家庭医生的了解程度越高、家庭没有发生灾难性卫生支出(OR=0.751)、有体育锻炼的农村居民(OR=1.200)对家庭医生预约转诊个性化签约服务的支付意愿越高,年轻人比老年人支付意愿高。结论:家庭医生预约转诊个性化签约服务个人筹资实施具有可行性。加强家庭医生签约服务的宣传力度,提升基层卫生保健医疗服务的水平,是提高农村居民对家庭医生预约转诊个性化签约服务支付意愿的有效方法。Objective:To explore the willingness to pay(WTP)for family doctor contracting services(FDCS)of appointment and referral and the influencing factors of rural residents in Shandong province by taking the Anderson theory as the framework,so as to provide reference for promoting the development of FDCS in China.Methods:A questionnaire survey was conducted among 2979 rural residents in Shandong province by multistage stratified random sampling.Results:A total of 2970 valid questionnaires were collected,and 44.8%of rural residents were willing to pay for appointment and referral personalized contracting services.Rural residents with higher understanding of family doctors,no catastrophic health expenditure(OR=0.751)and physical exercise(OR=1.200)had higher WTP for personalized contracting service of family doctor appointment and referral,and young people had higher willingness to pay than older adults.Conclusion:It is feasible to implement for personal funding-raising for appointment and referral personalized FDCS.Strengthening the publicity of the contracted services of family doctors and improving the level of grassroots health care services can improve the willingness of rural residents to pay for the personalized contracting services for appointment and referral.
