摘 要:目的探讨干扰素α(IFNα)、高迁移率族蛋白-1(HMGB1)等细胞因子在系统性红斑狼疮(SLE)发生、发展中的作用,以及其与SLE疾病活动度、临床症状的关联,评估其检测的临床应用价值。方法采用ELISA法检测SLE患者血清HMGB1及IFNα水平;采用免疫磁珠法检测白细胞介素(IL)-1β、IL-2、IL-4、IL-5及IL-6水平;评估活动组与非活动组血清IFNα、HMGB1水平的试验效能。结果SLE组患者IFNα、HMGB1水平均高于类风湿关节炎(RA)组(P<0.05),IL-2及IL-6水平均低于RA组(P<0.05),活动组患者IFNα、HMGB1、IL-6水平高于与非活动组患者(P<0.05);HMGB1与IFNα呈正相关(r=0.76,P=0.012),HMGB1、IFNα与SLEDAI呈正相关(r=0.65,P=0.013;r=0.41,P=0.031);HMGB1与细胞因子IL-1β、IL-4、IL-5相关,IFNα与IL-1β、IL-4相关。结论IFNα与HMGB1相互促进,参与了SLE的发生、发展,IFNα、HMGB1与SLE疾病活动度及发热、肾脏损害等临床表现均有关联,其检测对临床上区分活动组与非活动组SLE患者有较大的应用价值。Objective To investigate the active pathogenic mechanism of interferon alfa(INFα),high mobility group box 1(HMGB1)and cytokines in SLE patients,and to explore the relationship between HMGB1/INFαand SLE activity and clinical features,and access the clinical detection value of these indicators.Methods Serum HMGB1 and IFNαin SLE patients were detected by ELISA.Interleukin(IL)-1β,IL-2,IL-4,IL-5 and IL-6 were detected by immunomagnetic bead assay.To evaluate the efficacy of serum IFNαand HMGB1 levels in active and inactive groups.Results The levels of serum HMGB1,INFαin SLE patients were higher than that in RA patients(P<0.05),while the levels of serum IL-2,IL-6 in SLE patients were lower than that in RA patients(P<0.05).The levels of serum HMGB1,INFα,IL-6 were higher in active SLE patients than that in non-active patients(P<0.05).HMGB1 was positively related to IFNα(r=0.76,P=0.012),HMGB1,IFNαwere positively related to SLEDAI(r=0.65,P=0.013,r=0.41,P=0.031).HMGB1 was related to IL-1β,IL-4,IL-5,and IFNαwas related to IL-1β,IL-4.Conclusion INFαand HMGB1 promoted the autoimmune process in SLE.HMGB1/INFαwere related to SLE activity and clinical features such as fever and renal damage and had good clinical value in distinguishing between active and non-active SLE.
