摘 要:目的:分析儿童支气管异物的临床及影像学特点,探讨多层螺旋C T及后处理技术在儿童支气管异物诊断中的应用价值。方法:选取2019年1月—2020年12月在滕州市中心人民医院行多层螺旋CT(multislice spiral CT,MSCT)检查的儿童气道异物病例,回顾性分析90例外源性支气管异物患儿的一般情况、异物吸入史、异物特征(种类、滞留部位、滞留时间)及影像学特征。结果:多层螺旋CT诊断支气管异物且经支气管镜手术证实的外源性异物90例,好发于幼儿期(1~3岁),男女比为2.5:1,86例有异物吸入史,异物种类以花生最常见,左侧支气管异物41例,右侧支气管异物48例,双侧支气管异物1例,肺气肿、肺炎、肺不张是常见并发症。结论:多层螺旋CT及后处理技术可作为儿童支气管异物首选的影像学检查方法,可采用低剂量扫描方案,降低辐射剂量。Objective To analyze the clinical and imaging features of tracheobronchial foreign bodies in children,and to explore the application value of multislice spiral CT and post-processing technology in the diagnosis of tracheobronchial foreign bodies in children.Methods Children with airway foreign body who underwent MSCT examination in Tengzhou Central People's Hospital from January 2019 to December 2020 were selected.They were retrospectively analyzed with the general situation,inhalation history,foreign body characteristics(types,retention sites,retention time)and imaging features of 90 children with exogenous tracheobronchial foreign body.Results Tracheobronchial foreign bodies were diagnosed by MSCT and confirmed by bronchoscopic surgery in 90 cases,most of which occurred in early childhood(1~3 years old).The male to female ratio was 2.5:1,86 cases had a history of foreign body inhalation.Peanut was the most common type of foreign body.There were 41 cases of left bronchial foreign bodies,48 cases of right bronchial foreign bodies,1 case of bilateral bronchial foreign bodies.Emphysema,pneumonia and atelectasis were common complications.Conclusion The post-processing technology of MSCT can be used as the preferred imaging examination method for tracheobronchial foreign bodies in children.Low-dose scanning scheme can be used to reduce the radiation dose.
