沙库巴曲缬沙坦钠治疗老年慢性心力衰竭合并肾功能不全患者的临床效果

摘　　要：目的分析沙库巴曲缬沙坦钠治疗老年慢性心力衰竭合并肾功能不全患者的临床效果。方法选取120例老年慢性心力衰竭合并肾功能不全患者作为研究对象,依据各患者到院诊治顺序给予前60例患者贝那普利治疗,并将其设为对照组;给予后60例患者沙库巴曲缬沙坦钠治疗,并将其设为研究组。比较两组的临床效果。结果治疗后,研究组的LVEF、LVESD、LVEDD、LVPW、E/e'、室间隔速度e'、左室侧壁速度e'、三尖瓣反流速度优于对照组(P<0.05)。治疗后,研究组的心率、QRS波时限、QTc、P-R间期均低于对照组(P<0.05)。治疗后,两组的血清胱抑素C、血肌酐水平降低,eGFR升高,且研究组优于对照组(P<0.05)。治疗后,研究组的KCCQ-OSS-15评分高于对照组(P<0.05)。结论沙库巴曲缬沙坦钠不仅可有效控制老年慢性心力衰竭合并肾功能不全患者的病情进展,还可有效提高其心功能和肾功能指标,改善生存质量。Objective To analyze the clinical effect of sacubitril valsartan sodium in the treatment of elderly patients with chronic heart failure complicated with renal insufficiency.Methods A total of 120 elderly patients with chronic heart failure complicated with renal insufficiency were selected as the research objects.According to the order of diagnosis and treatment,the first 60 patients were treated with benazepril as control group,and the last 60 patients were treated with sacubitril valsartan sodium as study group.The clinical effects of the two groups were compared.Results After treatment,LVEF,LVESD,LVEDD,LVPW,E/e',ventricular septal velocity e',left ventricular sidewall velocity e',tricuspid regurgitation velocity in the study group were better than those in the control group(P<0.05).After treatment,the heart rate,QRS wave duration,QTc and P-R interval in the study group were lower than those in the control group(P<0.05).After treatment,the levels of serum cystatin C and serum creatinine in the two groups decreased,and eGFR increased,and those in the study group were better than the control group(P<0.05).After treatment,the score of KCCQ-OSS-15 in the study group was higher than that in the control group(P<0.05).Conclusion Sacubitril valsartan sodium can not only effectively control the disease progress of elderly patients with chronic heart failure complicated with renal insufficiency,but also effectively improve the cardiac and renal function indexes and ameliorate the quality of life.
