摘　　要：目的探讨介入栓塞对支气管扩张伴大咯血患者出血控制率的影响。方法选取2016年5月至2019年9月收治的73例支气管扩张伴大咯血患者作为研究对象,根据治疗方式不同将其分为观察组(37例)与对照组(36例)。观察组采取介入栓塞治疗,对照组采取内科保守治疗。比较两组的止血效果、不良反应发生情况、生活质量、实验室指标、出血时间及出血总量。结果观察组的止血成功率高于对照组(P<0.05)。两组的不良反应总发生率无显著差异(P>0.05)。治疗后,两组的SGRQ评分、hs-CRP水平、WBC及N%均降低,且观察组低于对照组(P<0.05)。治疗后,观察组的出血时间短于对照组,出血总量少于对照组(P<0.05)。结论介入栓塞术治疗支气管扩张伴大咯血患者效果显著,能有效控制出血量,改善炎症水平,提高生活质量,安全性高,值得临床应用推广。Objective To investigate the effects of interventional embolization on bleeding control rate in patients with bronchiectasis and massive hemoptysis.Methods A total of 73 patients with bronchiectasis and massive hemoptysis admitted in our hospital from May 2016 to September 2019 were selected as the research objects,and the patients were divided into observation group(37 cases)and control group(36 cases)according to different treatment methods.The observation group was treated with interventional embolization,and the control group was treated with medical conservative treatment.The hemostatic effect,adverse reactions,quality of life,laboratory indexes,bleeding time and total amount of bleeding were compared between the two groups.Results The success rate of hemostasis in the observation group was higher than that in the control group(P<0.05).There was no significant difference in the total incidence of adverse reactions between the two groups(P>0.05).After treatment,SGRQ score,hs-CRP level,WBC and N%of the two groups decreased,and those of the observation group were lower than the control group(P<0.05).After treatment,the bleeding time in the observation group was shorter than that in the control group,and the total amount of bleeding was less than that in the control group(P<0.05).Conclusion Interventional embolization is effective in the treatment of bronchiectasis and massive hemoptysis.It can effectively control the amount of bleeding,improve the level of inflammation and the quality of life,with high safety,which is worthy of clinical application and promotion.
