摘  要:目的探究早、中孕期产前超声筛查在胎儿神经系统畸形诊断中的应用价值。方法以1100例行胎儿神经系统畸形筛查孕妇为研究对象,并以染色体核型检查、引产尸检等结果为金标准,评价产前超声对胎儿神经系统畸形的诊断效能。结果产前超声共检出58例胎儿神经系统畸形。全前脑、露脑、脑及脑膜膨出、无脑儿等神经管畸形可在早孕期被检出,中孕期主要常见的神经系统畸形包括脑积水、脊柱裂等。产前超声诊断胎儿神经系统畸形的灵敏度为93.33%,特异度为99.81%,准确度为99.45%,阳性预测值为96.55%,阴性预测值为99.62%。结论早、中孕期产前超声筛查能够较好地对胎儿神经系统畸形进行鉴别诊断。Objective To explore the application value of prenatal ultrasound screening in early and middle pregnancy in the diagnosis of fetal nervous system malformations.Methods A total of 1100 pregnant women screened for fetal nervous system malformations were selected as the research objects.The diagnostic efficacy of prenatal ultrasound in fetal nervous system malformations was evaluated by taking the results of chromosome karyotype examination and induced labor autopsy as the gold standard.Results Prenatal ultrasound detected 58 cases of fetal nervous system malformations.Neural tube malformations such as total forebrain,exposed brain,encephalomeningocele and anencephaly can be detected in early pregnancy.The main common nervous system malformations in middle pregnancy included hydrocephalus,spina bifida and so on.The sensitivity,specificity,accuracy,positive predictive value and negative predictive value of prenatal ultrasound in the diagnosis of fetal nervous system malformations were 93.33%,99.81%,99.45%,96.55% and 99.62%,respectively.Conclusion Prenatal ultrasound screening in early and middle pregnancy can better identify and diagnosis fetal nervous system abnormalities.
