摘　　要：难治性发热病机较为复杂,肿瘤发热在难治性发热中具有代表性。史载祥教授认为,运用经方治疗难治性发热的关键在于精准辨证。临证时当从整体出发,明确邪正关系,处方之时当谨守病机,方证对应,既遵循中医学整体观念、辨证施治的基本特点,又强调更为精准的辨证与个体化治疗。史教授创立清解三阳退热法、培土生金宣热法、灌肠逐瘀导热法,治疗难治性发热。清解三阳退热法方用葛根汤加石膏合大柴胡汤,培土生金宣热法方用甘草干姜汤合麻杏石甘汤,灌肠逐瘀导热法方用桃核承气汤灌肠。The pathogenesis of refractory fever is complex,and tumor fever is representative in refractory fever.Professor Shi Zai-xiang believes that the key to the use of classical formulas in the treatment of refractory fever lies in accurate syndrome differentiation.In the clinical practice,we should base on the concept of holism and clarify the relationship between evils and healthy qi.When prescribing,we should abide by the pathogenesis and the correspondence between formulas and syndromes,not only follow the concept of holism of TCM and the basic characteristics of syndrome differentiation and treatment,but also emphasize more accurate syndrome differentiation and individualized treatment.Professor Shi has established the method of removing fever by clearing three-yang,the method of dispersing fever by banking up earth to benefit metal,and the method of conducting fever by edema to remove blood stasis.Pueraria Decoction plus Shigao(Gypsum Fibrosum)combined with Major Bupleurum Decoction,Licorice and Rhizoma Zingiberis Decoction combined with Ephedra and Apricot Kernel and Gypsum and Licorice Decoction,Peach Kernel Qi-Guiding Decoction for edema are often used respectively in the above methods to treat refractory fever.
