手外伤感染创面修复治疗中封闭式负压引流技术的应用价值分析

摘　　要：目的探讨手外伤感染创面修复治疗中封闭式负压引流技术(VSD)的应用价值。方法94例手外伤感染患者,随机分为观察组与对照组,各47例。对照组使用传统每日换药处理,观察组使用VSD治疗。比较两组创面修复优良率、治疗时间、各项治疗指标、并发症发生情况及治疗前后视觉模拟评分法(VAS)评分、生活质量评分量表(QOL)评分。结果观察组创面修复优良率为95.74%,高于对照组的74.47%,差异有统计学意义(P<0.05)。观察组抗感染药物使用时间、创面愈合时间、住院时间分别为(7.21±1.46)、(15.78±1.84)、(18.29±2.36)d,均短于对照组的(13.53±2.37)、(26.85±2.47)、(28.74±3.62)d,差异有统计学意义(P<0.05)。观察组一周愈合率、日引流量高于对照组,换药次数、二次手术率低于对照组,二期植皮时间短于对照组,差异均有统计学意义(P<0.05)。观察组并发症发生率为2.13%低于对照组的23.40%,差异有统计学意义(P<0.05)。治疗后,观察组VAS、QOL评分均优于对照组,差异有统计学意义(P<0.05)。结论手外伤感染创面修复治疗中VSD的应用价值显著,能缩短治疗时间,减轻疼痛感,降低二次手术率和并发症发生率,提高创面修复优良率,提升生活质量,具有积极的临床意义。Objective To discuss the application value of vacuum sealing drainage(VSD)in the repair of infected wounds of hand trauma.Methods A total of 94 patients with hand trauma infection were randomly divided into observation group and control group,with 47 cases in each group.The control group was treated with traditional daily dressing change,and the observation group was treated with VSD.Both groups were compared in terms of excellent rate of wound repair,treatment time,various treatment indicators,complication rate,visual analogue scale(VAS)score before and after treatment,and quality of life scale(QOL)score were compared between the two groups.Results The excellent rate of wound repair in the observation group was 95.74%,which was higher than 74.47%in the control group,and the difference was statistically significant(P<0.05).The time of anti-infective drug use,wound healing time,and hospitalization time in the observation group were(7.21±1.46),(15.78±1.84),and(18.29±2.36)d,which were shorter than(13.53±2.37),(26.85±2.47),and(28.74±3.62)d in the control group,and the differences were statistically significant(P<0.05).The one-week healing rate and daily drainage volume of the observation group were higher than those of the control group,the number of dressing changes and the secondary surgery rate were lower than those of the control group,and the second-stage skin grafting time was shorter than that of the control group.All the differences were statistically significant(P<0.05).The complication rates in the observation group 2.13%,which was lower than 23.40%in the control group,and the difference was statistically significant(P<0.05).After treatment,the VAS and QOL scores of the observation group were better than those of the control group,and the difference was statistically significant(P<0.05).Conclusion The application of VSD is of significant value in the treatment of infected wounds of hand trauma,which can shorten the treatment time,reduce pain,reduce the rate of secondary surgery and complications,impro
