血清C肽、糖化血红蛋白检测对糖尿病的诊断效能及餐后2h血清C肽水平的影响

摘　　要：目的:探讨血清C肽(CP)、糖化血红蛋白(HbA1c)检测对2型糖尿病(T2DM)的诊断效能及餐后2h血清C肽(2hP-CP)水平的影响。方法:选择2019年3月~2021年3月于某院治疗的75例T2DM患者作为T2DM组,同时选取同期治疗的70例糖尿病肾病(DN)患者作为DN组,选取同期体检健康者70例作为对照组。测定并比较3组空腹血糖(FPG)、餐后2h血糖(2hPG)水平及CP、HbA1c、2hP-CP水平,分析CP、HbA1c与2hP-CP相关性。结果:T2DM组及DN组FPG、2hPG水平高于对照组,HbA1c水平:DN组>T2DM组>对照组,CP、2hP-CP水平:DN组<T2DM组<对照组,有统计学差异(P<0.05);T2DM组与DN组FPG、2hPG水平对比,无统计学差异(P>0.05);经Pearson相关性检验结果显示,CP与2hP-CP呈正相关,HbA1c与2hP-CP呈负相关(P<0.05)。结论:CP、HbA1c与2hP-CP具有一定相关性,测定CP、HbA1c与2hP-CP水平能够为T2DM患者的诊断及病情严重程度提供参考,反映T2DM患者血糖控制水平。Objective:To investigate the diagnostic efficacy of serum C peptide(CP)and glycosylated hemoglobin(HbA1c)test in type 2 diabetes mellitus(T2DM)and the influence on serum C peptide(2hP-CP)level at 2h after meal.Methods:75 T2DM patients treated in a hospital from March 2019 to March 2021 were selected as T2DM group.Meanwhile,70 patients with diabetic nephropathy(DN)treated in a hospital during the same period were selected as the DN group,and 70 healthy persons in a hospital during the same period were selected as the control group.The fasting blood glucose(FPG),2 hours after meal glucose(2hPG)and CP,HbA1c,2hP-CP levels in the three groups were measured and compared,and the correlation between CP,HbA1c and 2hP-CP was analyzed.Results:The levels of FPG and 2hPG in T2DM group and DN group were higher than those of control group.HbA1c level:DN group>T2DM group>control group,and CP and 2hP-CP:DN grou P<T2DM grou P<control group,with statistical difference(P<0.05);There was no significant difference in FPG and 2hPG levels between T2DM group and DN group(P>0.05);The Pearson correlation test showed that CP was positively correlated with 2hP-CP,HbA1c was negatively correlated with 2hP-CP(P<0.05).Conclusion:CP,HbA1c and 2HP CP have certain correlation.The determination of CP,HbA1c and 2hP-CP can provide reference for the diagnosis and severity of T2DM patients,and reflect the blood glucose control level of T2DM patients.
