祛痰利肺汤加减对慢性阻塞性肺炎急性加重期咳喘的治疗效果及对炎性因子影响

摘　　要：目的:探讨慢性阻塞性肺炎急性加重期咳喘治疗中祛痰利肺汤加减治疗的效果及对炎性因子影响。方法:回顾性选取2019年3月~2021年3月某院门诊收治的慢性阻塞性肺炎急性加重期咳喘患者96例,依据治疗方法分为常规性止咳平喘治疗组和常规性止咳平喘治疗基础上祛痰利肺汤加减治疗组(祛痰利肺汤加减治疗组)各48例,统计分析两组患者的中医症状积分、临床疗效、肺功能、动脉血气、血清炎性因子水平。结果:治疗后,祛痰利肺汤加减治疗组患者的咳嗽、咳痰、喘息、气短积分均低于常规性止咳平喘治疗组(P<0.05)。祛痰利肺汤加减治疗组患者治疗的总有效率91.67%(44/48)高于常规性止咳平喘治疗组的72.92%(35/48)(P<0.05)。治疗后,祛痰利肺汤加减治疗组患者的FEV1%、PEF、PaO 2均高于常规性止咳平喘治疗组(P<0.05),PaCO_(2)低于常规性止咳平喘治疗组(P<0.05)。治疗后,祛痰利肺汤加减治疗组患者的血清IL-8、TNF-α水平均低于常规性止咳平喘治疗组(P<0.05)。结论:慢性阻塞性肺炎急性加重期咳喘治疗中祛痰利肺汤加减治疗的效果好,能够有效降低患者的血清炎性因子水平。Objective:To investigate the effect of Qutan Lifei Decoction and its influence on inflammatory factors in the treatment of acute exacerbation of chronic obstructive pneumonia.Methods:96 cases of patients with cough and asthma in acute exacerbation of chronic obstructive pneumonia in a hospital from March 2019 to March 2021 were selected retrospectively.According to the treatment methods,they were divided into two groups:conventional cough and asthma relieving treatment group and Qutan Lifei decoction plus conventional cough and asthma relieving treatment group(Qutan Lifei decoction treatment group),with 48 cases in each group.The TCM symptom scores,clinical efficacy,pulmonary functions,arterial blood gas and serum inflammatory factor levels of the two groups were statistically analyzed.Results:After treatment,the scores of cough,expectoration,wheezing and shortness of breath in the Qutan Lifei decoction treatment group were lower than those in the conventional cough and asthma relieving treatment group(P<0.05).The total effective rate of the Qutan Lifei decoction treatment group was 91.67%(44/48),which was higher than 72.92%(35/48)of the conventional cough and asthma treatment group(P<0.05).After treatment,the FEV1%,PEF and PaO 2 in the Qutan Lifei decoction treatment group were higher than those in the conventional cough and asthma relieving treatment group(P<0.05),the PaCO 2 in the Qutan Lifei decoction treatment group was lower than that in the conventional cough and asthma relieving treatment group(P<0.05).After treatment,the serum levels of IL-8 and TNF-αin the Qutan Lifei decoction treatment group were significantly lower than those in the conventional cough and asthma relieving treatment group(P<0.05).Conclusion:The effect of Qutan Lifei decoction treatment group in the treatment of acute exacerbation of chronic obstructive pneumonia cough and asthma is good,which can effectively reduce the level of serum inflammatory factors.
