摘  要:目的探讨2型糖尿病患者尿白蛋白排泄率(UAER)与患者血清1型胶原蛋白氨基端前胶原肽(P1NP)、骨钙素(BGP)、25-羟维生素D_(3)[25(OH)D_(3)]水平的相关性。方法回顾性分析2020年1~12月安康市中医医院收治的182例2型糖尿病患者的临床资料,按UAER水平不同分为大量白蛋白尿组(UAE>300 mg/g)40例,微量白蛋白尿组(UAER 30~300 mg/g)62例和正常白蛋白尿组(UAER<30 mg/g)80例。比较三组患者的糖尿病病程、空腹血糖(FBG)、糖化血红蛋白(HbA1c)、血肌酐(SCr)、血尿素氮(BUN)、血清P1NP、BGP、25(OH)D_(3)水平,并采用Pearson法分析UAER和各指标的相关性。结果大量白蛋白尿组患者的糖尿病病程、FBG、HbA1c、SCr、BUN水平分别为(11.58±2.74)年、(10.12±1.37)mmol/L、(10.33±1.29)%、(77.30±5.96)μmol/L、(6.86±0.81)mmol/L,明显长于/高于微量蛋白尿组的(8.32±2.22)年、(8.90±1.14)mmol/L、(8.85±1.35)%、(68.28±5.61)μmol/L、(6.01±1.05)mmol/L和正常白蛋白尿组的(6.64±1.39)年、(7.74±1.08)mmol/L、(7.48±1.26)%、(60.55±6.94)μmol/L、(5.09±0.72)mmol/L,且微量蛋白尿组患者上述指标均明显长于/高于正常白蛋白尿组,差异均有统计学意义(P<0.05);大量白蛋白尿组患者血清P1NP、BGP、25(OH)D_(3)水平分别为(40.92±4.06)ng/mL、(7.83±1.08)ng/mL、(51.47±6.84)nmol/L,明显低于微量蛋白尿组的(47.71±4.78)ng/mL、(9.30±1.69)ng/mL、(63.33±4.91)nmol/L和正常白蛋白尿组的(56.83±5.23)ng/mL、(12.46±2.11)ng/mL、(72.80±5.04)nmol/L,微量蛋白尿组患者上述指标明显低于正常白蛋白尿组,差异均有统计学意义(P<0.05);经Pearson相关性分析结果显示,UAER与糖尿病病程、FBG、HbA1c、SCr、BUN均呈正相关(P<0.05),与P1NP、BGP、25(OH)D_(3)均呈负相关(P<0.05)。结论2型糖尿病患者UAER与患者血清P1NP、BGP、25(OH)D_(3)水平具有明显相关性。Objective To study the correlation between urinary albumin excretion rate(UAER)and serum Nterminal procollagen peptide of type 1 collagen(P1NP),osteocalcin(BGP),25hydroxyvitamin D_(3)[25(OH)D_(3)]levels in patients with type 2 diabetes mellitus.Methods The clinical data of 182 patients with type 2 diabetes,who admitted to the Traditional Chinese Medicine Hospital of Ankang City from January 2020 to December 2020,were retrospectively analyzed.The patients were divided into three groups according to UAER levels:40 patients with massive albuminuria group(UAER>300 mg/g),62 patients with microalbuminuria group(UAER 30 mg/g to 300 mg/g)and 80 patients with normal albuminuria group(UAER<30 mg/g).The course of diabetes,fasting blood glucose(FBG),glycosylated hemoglobin(HbA1c),serum creatinine(SCr),blood urea nitrogen(BUN),serum P1NP,BGP and 25(OH)D_(3)levels were compared among the three groups,and the Pearson method was used to analyze the correlation between UAER and each index.Results The course of diabetes,FBG,HbA1c,SCr,BUN levels in the massive albuminuria group were(11.58±2.74)years,(10.12±1.37)mmol/L,(10.33±1.29)%,(77.30±5.96)μmol/L,(6.86±0.81)mmol/L,respectively,which were significantly longer/higher than corresponding(8.32±2.22)years,(8.90±1.14)mmol/L,(8.85±1.35)%,(68.28±5.61)μmol/L,(6.01±1.05)mmol/L in microalbuminuria group and corresponding(6.64±1.39)years,(7.74±1.08)mmol/L,(7.48±1.26)%,(60.55±6.94)μmol/L,(5.09±0.72)mmol/L in normal albuminuria group;the above indexes in microalbuminuria group were significantly longer/higher than those in normal albuminuria group;the differences were statistically significant(all P<0.05);the serum P1NP,BGP and 25(OH)D_(3)levels in the massive albuminuria group were(40.92±4.06)ng/mL,(7.83±1.08)ng/mL,(51.47±6.84)nmol/L,respectively,which were significantly lower than corresponding(47.71±4.78)ng/mL,(9.30±1.69)ng/mL,(63.33±4.91)nmol/L in microalbuminuria group,and corresponding(56.83±5.23)ng/mL,(12.46±2.11)ng/mL,(72.80±5.04)nmol/L in normal albuminuria
