参芪地黄汤联合常规西药治疗肾性贫血患者的效果

摘　　要：目的:观察参芪地黄汤联合常规西药治疗肾性贫血(RA)患者的效果。方法:回顾性分析2018年3月至2021年3月该院收治的84例RA患者的临床资料,按照治疗方法不同分为对照组和观察组各42例。对照组给予常规西药治疗,观察组在对照组基础上联合参芪地黄汤治疗,比较两组临床疗效,治疗前后肾功能指标水平,血清铁调素、低氧诱导因子-1α(HIF-1α)、脯氨酰羟化酶(PHD)水平和不良反应发生率。结果:观察组治疗总有效率为92.86%(39/42),高于对照组的71.43%(30/42),差异有统计学意义(P<0.05);治疗后,观察组血肌酐、尿素氮、血清铁调素、HIF-1α和PHD水平低于对照组,内生肌酐清除率高于对照组,差异均有统计学意义(P<0.05);两组不良反应发生率比较,差异无统计学意义(P>0.05)。结论:参芪地黄汤联合常规西药治疗RA患者,可提高治疗总有效率,改善肾功能指标水平,降低血清铁调素、HIF-1α和PHD水平,效果优于单纯常规西药治疗。Objective:To observe effects of Shenqi Dihuang decoction combined with conventional western medicine on renal anemia(RA).Methods:The clinical data of 84 RA patients admitted to this hospital from March 2018 to March 2021 were retrospectively analyzed.According to different treatment methods,they were divided into control group and observation group,each with 42 cases.The control group was treated with conventional western medicine,while the observation group was treated with Shenqi Dihuang decoction on the basis of that of the control group.The clinical efficacy,the levels of renal function indexes before and after the treatment,the levels of serum hepcidin,hypoxia inducible factor-1α(HIF-1α),prolyl hydroxylase(PHD)and the incidence of adverse reactions were compared between the two groups.Results:The total effective rate of treatment in the observation group was 92.86%(39/42),which was higher than 71.43%(30/42)in the control group,and the difference was statistically significant(P<0.05).After the treatment,the levels of blood creatinine,urea nitrogen,serum hepcidin,HIF-1αand PHD of the observation group were lower than those of the control group;the endogenous creatinine clearance rate was higher than that of the control group;and the differences were statistically significant(P<0.05).However,there was no statistically significant difference in the incidence of adverse reactions between the two groups(P>0.05).Conclusions:Shenqi Dihuang decoction combined with conventional western medicine in the treatment of the RA patients can increase the total effective rate of treatment,improve the levels of renal function indexes,and reduce the levels of serum hepcidin,HIF-1αand PHD.Moreover,it is superior to single conventional western medicine treatment.
