改良人字形平卧位在超低位直肠癌患者腹腔镜保肛根治术中的应用效果

摘　　要：目的:观察改良人字形平卧位在超低位直肠癌患者腹腔镜保肛根治术中的应用效果。方法:回顾性分析2018年1月至2020年8月该院收治的86例超低位直肠癌患者的临床资料,根据手术体位不同分为观察组和对照组各43例。对照组采用改良式低截石位,观察组采用改良人字形平卧位。比较两组体位摆放时间、体位舒适度及并发症情况。结果:观察组体位摆放时间明显短于对照组,体位舒适度高于对照组,差异均有统计学意义(P<0.05);观察组术后并发症发生率为4.65%,低于对照组的37.21%,差异有统计学意义(P<0.05)。结论:改良人字形平卧位用于超低位直肠癌患者腹腔镜保肛根治手术中,可缩短体位摆放时间,提高体位舒适度,降低并发症发生率,效果优于改良式低截石位。Objective:To observe application effects of modified herringbone supine position in laparoscopic anus-preserving radical resection of ultra-low rectal cancer.Methods:The clinical data of 86 patients with ultra-low rectal cancer admitted to our hospital from January 2018 to August 2020 were retrospectively analyzed.According to different surgical positions,they were divided into observation group and control group,43 cases in each group.The control group used the modified low lithotomy position,while the observation group used the modified herringbone supine position.The posture time,the posture comfort and the complications were compared between the two groups.Results:The posture time of the observation group was significantly shorter than that of the control group;and the posture comfort was higher than that of the control group;and the differences were statistically significant(P<0.05).Further,the incidence of postoperative complications in the observation group was 4.65%,which was lower than 37.21%in the control group,and the difference was statistically significant(P<0.05).Conclusions:The modified herringbone supine position used in laparoscopic anus-preserving radical resection for the patients with ultra-low rectal cancer can shorten the posture time,improve the posture comfort,and reduce the risk of complications.Moreover,it is superior to the modified lithotomy position.
