摘　　要：目的分析重症监护病房下呼吸道感染病原菌的分布及耐药性。方法选取2018年1—12月辽宁省大连市中心医院ICU病房院内感染的566例下呼吸道感染患者,分析下呼吸道感染细菌的分布特点及耐药性。结果通过细菌培养,共分离出977株病原菌。检出前5位的病原菌分别为鲍曼不动杆菌,肺炎克雷伯菌,铜绿假单胞菌,金黄色葡萄球菌和嗜麦芽窄食单胞菌,其中耐碳青酶烯的鲍曼不动杆菌及铜绿假单胞菌分别占比84.9%和37.6%,金黄色葡萄球菌中耐甲氧西林(MRSA)占比71.7%。结论ICU耐药形势比较严峻,应合理使用抗菌药物,减少多重耐药菌群,降低患者死亡率。Objective To analyze the distribution and drug resistance of pathogens of lower respiratory tract infection in intensive care unit.Methods 566 patients with lower respiratory tract infection in ICU ward of Dalian Central Hospital,Liaoning Province,from January to December 2018 were selected to analyze the distribution characteristics and drug resistance of bacteria in lower respiratory tract infection.Results There were 977 strains with the respiratory tract infection in ICU.The first 5 drug-resistant bacterias were acinetobacter baumanni,klebsiella pneumonia,pseudomonas aeruginosa,staphylococcus aureus and stenotrophomonas,respectively.The top 5 pathogens detected were Acinetobacter baumannii,Klebsiella pneumoniae,Pseudomonas aeruginosa,Staphylococcus aureus,and Stenotrophomonas maltophilia.Among them,carbapenem-resistant Acinetobacter baumannii and Pseudomonas aeruginosa accounted for 84.9%and 37.6%,respectively,and the methicillin-resistant(MRSA)rate of Staphylococcus aureus was 71.7%.Conclusion The drug resistance is serius.We should use the antibiotics reasonably,reduce the occurrence of infection and decrease the mortality rate of injured patients.
