基于突触可塑性探讨益髓复用汤治疗阿尔茨海默病

摘　　要：阿尔茨海默病最早出现的症状为记忆力下降,且贯穿疾病始终。突触可塑性中长时程增强(long-term potentiation,LTP)和长时程抑制(long-term depression,LTD)相互消长转化是学习记忆的基本条件。阿尔茨海默病初期,突触可塑性无明显变化,治宜温阳补气为主;中期,LTP持续受损、减少,突触可塑性遭到破坏,如脾胃虚弱,致痰饮内阻于脑,治宜滋阴益阳,化痰祛饮;后期,因淀粉样蛋白等代谢产物沉积过多,产生神经毒素,加重脑萎缩,如郁而成瘀,痰瘀互结,瘀久化热,热毒侵脑,热盛伤阴,阴阳俱损,最终脑络失养,治宜益精补髓,解毒化瘀。益髓复用汤可益精填髓补其形之阴,温阳益气复其阳之用,兼顾化痰祛瘀通络,对突触可塑性三期病变均可兼顾。The earliest symptom of Alzheimer′s disease(AD)is memory loss,which runs through from beginning to end.The mutual growth and decline change of long-term potentiation(LTP)and long-term depression(LTD)of synaptic plasticity is the basic condition of learning and memory.In the early stage of AD,synaptic plasticity has no significant change,so the treatment should mainly focus on warming yang and replenishing qi.In the middle stage,LTP continues to be damaged and reduced,and synaptic plasticity is destroyed.For example,the weakness of the spleen and stomach causes the internal obstruction of phlegm retention in the brain.So it is appropriate to nourish yin,warm yang,dissipate phlegm and dispel retention.In the later stage,due to the excessive deposition of amyloid and other metabolites,neurotoxins are produced and brain atrophy is aggravated,such as depression transforms into blood stasis;phlegm and blood stasis are intertwined;long-time blood stasis turns into heat;heat toxin invades the brain;heat accumulates and injures yin;yin and yang are both damaged;and finally the brain collaterals are out of nourishment.Therefore,it is advisable to nourish essence,replenish marrow,detoxify and remove blood stasis.Marrow-Nourishing and Reuse Decoction can supplement yin form and restore yang function,as well as resolve phlegm,remove blood stasis and dredge collaterals,and affect the three-stage lesions of synaptic plasticity.
