社会支持和心理韧性对血液透析患者自我感受负担的影响

摘　　要：目的探讨社会支持和心理韧性对血液透析患者自我感受负担的影响。方法选择2020年5月至10月在我院接受血液透析治疗的98例患者作为本次研究对象,随后采用一般资料调查表、社会支持评定量表(SSRS)、心理韧性量表(CD-RISC)以及自我感受负担量表(SPBS)对本次研究对象进行问卷调查。采用Pearson相关分析探讨社会支持及心理韧性与自我感受负担相关性,同时采用分层回归分析影响血液透析患者自我感受负担的相关因素。结果本次研究共发放98份问卷调查表,维持性血液透析患者自我感受负担得分为(27.58±5.22)分,处于轻度水平,其中无明显负担15例(15.31%),轻度负担19例(19.39%),中度负担38例(38.78%),重度负担26例(26.53%)。经维度分析显示:患者自我感受负担得分以身体负担为主,依次为情感负担、经济负担;Pearson相关性分析显示:自我感受负担各维度以及总分与社会支持及心理韧性总分均呈负相关(P<0.05)。回归分析显示:心理韧性、社会支持均是维持性血液透析患者自我感受负担的影响因素(P<0.05)。结论维持性血液透析患者大部分存在不同程度的自我感受负担,且严重程度与心理韧性、社会支持均呈负相关,因此相关医护人员可通过社会支持与心理韧性方面制定降低患者自我感受负担水平的干预措施。Objective To explore the effects of social support and psychological resilience on self perceived burden of hemodialysis patients.Methods 98 patients who received hemodialysis in our hospital from May 2020 to October 2020 were selected as the subjects of this study,and were investigated by general information questionnaire,social support scale(SSRs),Psychological Resilience Scale(cd-risc)and self perceived burden scale(SBPs).Pearson correlation analysis was used to explore the correlation between social support,psychological resilience and self perceived burden.At the same time,hierarchical regression analysis was used to analyze the related factors affecting self perceived burden of hemodialysis patients.Results a total of 98 questionnaires were distributed in this study.The self perceived burden score of maintenance hemodialysis patients was(27.58±5.22),which was at the mild level.There were no obvious burden in 15 cases(15.31%),mild burden in 19 cases(19.39%),moderate burden in 38 cases(38.78%)and severe burden in 26 cases(26.53%).The dimensional analysis showed that the score of patients self feeling burden was mainly physical burden,followed by emotional burden and economic burden.Pearson correlation analysis showed that all dimensions and total scores of self perceived burden were negatively correlated with the total scores of social support and psychological resilience(P<0.05).Regression analysis showed that psychological resilience and social support were the influencing factors of self perceived burden in maintenance hemodialysis patients(P<0.05).Conclusion Most maintenance hemodialysis patients have different degrees of self feeling burden,and the severity is negatively correlated with psychological resilience and social support.Therefore,relevant medical staff can formulate intervention measures to reduce the level of self feeling burden through social support and psychological resilience.
