类风湿关节炎合并骨质疏松症及骨折的研究进展

摘　　要：类风湿关节炎(rheumatoid arthritis,RA)是一种慢性全身性自身炎症性疾病,其特征是多个外周关节的关节炎导致显著的功能障碍及关节疼痛。骨质疏松症(osteoporosis,0P)是RA的主要合并症之一,可引起患者腰背酸痛、脊柱变形。骨质疏松性骨折(osteoporosis fractures,OPF)是OP最常见的严重并发症。目前RA合并OP、OPF正引起全世界的关注,其风险评估及治疗耗费了大量的医疗资源,不仅影响了RA患者疾病的转归,还增加了家庭和社会的经济负担。该文主要从RA合并0P、OPF的危险因素以及治疗的最新研究进展等方面进行综述,以期今后临床医生对RA合并OP、OPF进行大样本调研,为构建RA合并OP、OPF风险评估工具提供参考依据,以此来识别OP、OPF高危人群并对其采取积极有效的预防和治疗措施。Rheumatoid arthritis(RA)is a chronic systemic autoimmune disease,which is characterized by arthritis of multiple peripheral joints,resulting in significant dysfunction and joint pain.Osteoporosis(OP)is one of the main complications of RA,which can cause low back pain and spinal deformation.Osteoporotic fractures(OPF)is the most common serious complication of OP.At present,RA combined with OP and OPF is attracting worldwide attention,and its risk assessment and treatment has consumed a lot of medical resources,which not only affects the prognosis of RA patients,but also increases the economic burden of families and society.This paper reviews the risk factors of RA and the latest research progress of treatment in order to provide a reference basis for the construction of risk assessment tools,identify the high-risk groups of OP and OPF and take active and effective prevention and treatment measures.
