中医药干预AGEs-RAGE信号通路改善糖尿病肾病的机制研究概述

摘　　要：糖尿病肾病发病机制复杂,晚期糖基化终末产物(advanced glycosylation end products,AGEs)-糖基化终末产物受体(receptor of AGEs,RAGE)信号通路是糖尿病肾病的重要发病通路之一,也是近年来的研究热点。中医药治疗糖尿病肾病有一定的优势,研究表明中医药可通过干预AGEs-RAGE信号通路改善糖尿病肾病患者的临床症状,延缓病情进展。中医药通过抑制AGEs形成,减少AGEs-RAGE轴的激活,减少细胞外基质积聚、延缓肾纤维化,保护肾小球滤过膜,减轻糖尿病肾病病理损伤;中医药抑制RAGE表达,减轻AGEs-RAGE轴及其下游信号通路对肾脏的损伤;中医药阻断了氧化应激与AGEs-RAGE轴之间的的反馈通路,拮抗氧化应激,降低活性氧(reactive oxygen species,ROS)等氧化物水平,同时可增加超氧化物歧化酶(superoxide dismutase,SOD)、过氧化氢酶(catalase,CAT)等抗氧化指标,保护肾功能;中医药抑制AGEs-RAGE信号通路激活的炎症通道,抑制肾脏炎性反应,改善肾损伤;中医药改善AGEs-RAGE介导的内皮细胞损伤,减缓肾损害进展。本文就中医药干预AGEs-RAGE信号通路治疗糖尿病肾病的主要机制进行综述。The pathogenesis of diabetic nephropathy(DN)is complex,AGEs-RAGE signaling pathway is one of the important pathogenesis of DN,and is also a hot research issue in recent years.Traditional Chinese medicine has certain advantages in treating DN,studies have shown that traditional Chinese medicine can improve the clinical symptoms of DN patients and delay the progression of the disease by intervening the AGEs-RAGE signaling pathway.Traditional Chinese medicine can inhibit the formation of AGEs,reduce the activation of AGEs-RAGE axis,reduce the accumulation of extracellular matrix,delay the renal fibrosis,protect the glomerular filtration membrane,and alleviate the pathological damage of DN.Traditional Chinese medicine can inhibit the expression of RAGE and alleviate renal injury caused by AGEs-RAGE axis and its downstream signaling pathway.Traditional Chinese medicine blocks the feedback pathway between oxidative stress and AGEs-RAGE axis,antagonizes oxidative stress,reduces the levels of ROS and other oxides,and increases SOD at the same time,CAT and other antioxidant indexes to protect renal function.Traditional Chinese medicine inhibits inflammatory channels activated by AGEs-RAGE signaling pathway,inhibits renal inflammatory response,and improves renal injury.Traditional Chinese medicine can improve AGEs-RAGE mediated endothelial cell injury and slow down the progression of renal damage.In this paper,the main mechanism of traditional Chinese medicine intervening DN by AGEs-RAGE signaling pathway was summarized.
