甘草酸苷联合热毒宁治疗手足口病患儿的临床疗效及对免疫反应的影响

摘　　要：目的:探讨甘草酸苷联合热毒宁治疗手足口病(HFMD)患儿的临床疗效及对免疫反应的影响。方法:选取110例HFMD患儿为研究对象,按照治疗方式不同分为对照组和联合组,每组各55例。对照组在常规治疗基础上予以热毒宁治疗;联合组在常规基础上给予热毒宁联合甘草酸苷治疗;疗程均为5 d。比较两组患者临床疗效、淋巴细胞亚群(CD_(3)^(+)CD_(4)^(+)T细胞、CD_(3)^(+)CD_(8)^(+)T细胞及CD_(4)^(+)/CD_(8)^(+))、NK细胞水平、炎症因子[中性粒细胞百分比(NEU%)、C反应蛋白(CRP)]水平、心肌酶[乳酸脱氢酶(LDH)、肌酸磷酸激酶(CK)、肌酸磷酸激酶同工酶(CK-MB)]水平及不良反应。结果:联合组临床总有效率高于对照组(P<0.05)。联合组患者发热、疱疹、溃疡和神经系统受累症状消退时间短于对照组(P<0.05)。治疗后,两组患者CD_(3)^(+)CD_(4)^(+)T细胞、CD_(3)^(+)CD_(8)^(+)T细胞、CD_(4)^(+)/CD_(8)^(+)及NK细胞水平均升高(P<0.05),且联合组高于对照组(P<0.05);NEU%、CRP水平均下降(P<0.05),且联合组低于对照组(P<0.05);LDH、CK-MB、CK水平均降低(P<0.05),且联合组低于对照组(P<0.05)。两组患者不良反应发生率比较,差异无统计学意义(P>0.05)。结论:与热毒宁治疗相比,甘草酸苷联合热毒宁治疗HFMD患儿临床疗效更加明显,可以更好地降低炎症与免疫反应,改善心肌酶水平,且使用安全,值得临床推广。Objective:To investigate the clinical efficacy of Reduning combined with glycyrrhizin in the treatment of children with hand-foot-mouth disease(HFMD) and its effect on inflammatory immune reaction.Methods:110 children with HFMD were divided into control group(n=55) and combined group(n=55) according to different treatment methods.The control group was treated with Reduning on the basis of routine treatment,and the combined group was treated with Reduning combined with glycyrrhizin on the basis of routine treatment.5 days was a course of treatment,and both groups were treated for 1 course.The clinical efficacy,lymphocyte subsets [CD_(3)^(+)CD_(4)^(+)T cells,CD_(3)^(+)CD_(8)^(+)T cells,CD_(4)^(+)/CD_(8)^(+)] and NK cell levels,inflammatory indicators [neutrophil %(NEU%),C-reactive protein(CRP)],cardiac enzymes [lactate dehydrogenase(LDH),creatine phosphate kinase(CK),creatine phosphokinase isoenzyme(CK-MB)] and adverse reactions were compared between the two groups.Results:The total clinical effective rate in combined group was significantly higher than that in control group(P<0.05).The time of symptoms of fever,herpes,ulcer and nervous system involvement in combined group were shorter those in control group(P<0.05).After treatment,the levels of CD_(3)^(+) CD_(4)^(+) T cells,CD_(3)^(+) CD_(8)^(+) T cells,CD_(4)^(+)/CD_(8)^(+) and NK cells were significantly increased in the two group(P<0.05),and the combined group was higher than the control group(P<0.05).The levels of NEU% and CRP in both groups decreased(P<0.05),and the combined group was lower than the control group(P<0.05).The levels of LDH,CK-MB and CK in the two groups decreased(P<0.05),and the combined group were lower than the control group(P<0.05).There was no significant difference in the incidence of adverse reactions between the two groups(P>0.05).Conclusion:Compared with Reduning,Reduting combined with glycyrrhizin has obvious clinical efficacy in the treatment of HFMD children,which can better reduce inflammatory immune response and improve myocardial
