摘  要:目的:探讨电刺激联合生物反馈治疗产后压力性尿失禁(SUI)的疗效。方法:选取80例产后SUI患者为研究对象,按照治疗方式不同分为对照组和观察组,每组各40例。对照组进行盆底肌肉训练治疗(Kegel运动);观察组在对照组基础上增加电刺激联合生物反馈治疗,疗程均为8周。观察两组临床疗效,比较治疗前后盆底肌力(手测肌力、Ⅰ类肌纤维最大值、Ⅱ类肌纤维平均值)、盆底功能[尿失禁问卷表(ICI-Q-SF)、女性性功能指数(FSFI)]、尿流动力学[腹压漏尿点压(ALPP)、最大尿道闭合压力(MUCP)、最大尿流率(Qmax)]、盆底肌电值[盆底最大肌电压(Dmax)、盆底肌持续收缩60 s平均肌电压(H60s)]。结果:观察组临床总有效率高于对照组(P<0.05)。治疗后,观察组手测肌力、Ⅰ类肌纤维最大值、Ⅱ类肌纤维平均值、FSFI评分及ALPP、MUCP、Qmax、Dmax、H60s高于对照组(P<0.05);ICI-Q-SF评分低于对照组(P<0.05)。结论:电刺激联合生物反馈治疗产后SUI可提高临床疗效,增强患者盆底肌力和盆底功能,改善尿流动力学、盆底肌电值。Objective:To investigate the efficacy of electrical stimulation combined with biofeedback in the treatment of postpartum stress urinary incontinence(SUI).Methods:80 patients with postpartum SUI were selected and divided into the control group and the observation group according to different treatment methods,with 40 cases in each group.The control group was treated with pelvic floor muscle training(Kegel exercise),and the observation group was treated with electrical stimulation combined with biofeedback on the basis of the control group.Both groups were treated for 8 weeks.The clinical efficacy of the two groups was observed,and the pelvic floor muscle strength [hand-measured muscle strength,the maximum value of type I muscle fiber and the average value of type II muscle fiber],pelvic floor function [urinary incontinence questionnaire(ICI-Q-SF),female sexual function index(FSFI)],urodynamics [abdominal pressure urine leakage point pressure(ALPP),maximum urethral closure pressure(MUCP),maximum urinary flow rate(Qmax)],pelvic floor EMG [maximum pelvic floor muscle voltage(Dmax),average pelvic floor muscle voltage for 60's of continuous contraction(H60 s) ] were compared before and after treatment.Results:The total effective rate of the observation group was significantly higher than that of the control group(P<0.05).Compared with the control group,the hand muscle strength,the maximum value of type I muscle fiber,the average value of type II muscle fiber,FSFI score,ALPP,MUCP,Qmax,Dmax,and H60's in the observation group were significantly higher,and the ICI-Q-SF score was significantly lower,with significant differences between groups(P<0.05).Conclusion:Electrical stimulation combined with biofeedback in the treatment of postpartum SUI can significantly improve clinical efficacy,enhance pelvic floor muscle strength and pelvic floor function,improve urodynamics and pelvic floor electromyography.
