县级医院开展成人腹腔镜腹股沟疝日间无张力修补手术的应用研究

摘　　要：目的:探讨在县级医院开展成人腹腔镜腹股沟疝日间无张力修补手术的临床应用效果。方法:2017年9月-2020年6月收治单侧腹股沟疝患者62例,分为两组,各31例。试验组在日间管理模式下实施成人腹腔镜腹股沟疝无张力修补手术,对照组则应用住院管理模式,比较两组临床效果。结果:试验组住院时间及医疗费用均显著低于对照组,差异有统计学意义(P<0.05);两组手术时间及并发症发生率比较,差异无统计学意义(P>0.05)。结论:县级医院开展成人腹腔镜腹股沟疝无张力修补手术,日间管理模式手术和住院管理模式手术效果相当,但日间管理模式手术的医疗费用低,周转快,且安全有效,提高了医疗资源的利用率,尤其减轻了患者经济负担及国家医疗负担。Objective:To explore the clinical application effect of adult laparoscopic daytime tension-free repair of inguinal hernia in county-level hospitals.Methods:From September 2017 to June 2020,62 patients with unilateral inguinal hernia were selected.They were divided into two groups with 31 cases in each group.In the experimental group,adult laparoscopic tension-free inguinal hernia repair was performed under daytime management mode.The control group was treated with hospitalization management mode.The clinical effects of the two groups were compared.Results:In the experimental group,the length of hospital stay and medical expenses were significantly lower than those in the control group,the difference was statistically significant(P<0.05).There was no significant difference in operation time and complication rate between the two groups(P>0.05).Conclusion:Adult laparoscopic tension-free inguinal hernia repair carried out in county-level hospitals achieve similar surgical effect at daytime management mode and hospitalization management mode,however,the medical cost of daytime management mode surgery is lower,the turnover is fast,it is safe and effective,and it can improve the utilization of medical resources,especially reduce the economic burden of patients and the national medical burden.
