摘  要:目的观察大学生不同血压水平者血钠、血钾、血钙水平与血压的相关性。方法采用随机和整群抽样结合的方法抽取郑州市三所大学的学生进行血压调查,按照《中国高血压防治指南(2018年修订版)》的标准,将大学生分为正常血压、正常高值及高血压3组。然后采用R语言分层抽样的方法抽取正常血压者83人,正常高值者101人,高血压患者102例。以上抽取的人群禁食12 h后,分批次到河南省人民医院体检中心采空腹静脉血,使用西门子临床化学系统(ADVIA2400)进行血电解质及生化测定。结果血钙、血钠/血钙在正常血压、正常高值及高血压三组之间差异有统计学意义(P<0.05)。高血压组血钙水平高于正常血压和正常高值组[(2.48±0.11)比(2.36±0.12)比(2.41±0.26)mmol/L,F=9.810,P<0.001]。相关分析显示,血钠(r_(s)=0.139,P=0.019)、血钙(r_(s)=0.377,P<0.001)与收缩压呈正相关,血钠/血钙(r=-0.354,P<0.001)与收缩压呈负相关;血钠(r_(s)=0.140,P=0.018)、血钙(r=0.301,P<0.001)与舒张压呈正相关,血钠/血钙与舒张压呈负相关(r=-0.264,P<0.001)。多元逐步线性回归分析显示:大学生收缩压与体质量指数(BMI)、血钠、血钙呈正相关(分别β=0.304,P=0.028;β=0.121,P=0.013;β=0.336,P<0.001);舒张压与BMI、血钠、血钙亦呈正相关(分别β=0.130,P=0.032;β=0.124,P=0.021;β=0.273,P<0.001)。结论大学生高血压患者血钙水平高于非高血压者,血钙与收缩压、舒张压均有相关性。Objective To analyse the correlation between plasma sodium,potassium,calcium and blood pressure levels in college students.Methods Students from three universities in Zhengzhou were randomly selected and cluster-sampled for blood pressure survey.According to the"Chinese guidelines for prevention and treatment of hypertension(revised in 2018)",the students were divided into three groups:normal blood pressure,normal high blood pressure and hypertension group.Eighty-three,101 and 102 cases were then selected respectively by stratified sampling using R language from each group.After fasting for 12 hours,fasting venous blood was collected from the physical examination center of Henan Provincial People’s Hospital in batches.The plasma electrolytes and biochemical parameters were determined by Siemens clinical chemistry system(ADVIA2400).Results There were significant differences in blood calcium and blood sodium to calcium ratio among the three groups(P<0.05).The blood calcium level in hypertension group was higher than that in normal blood pressure group or normal high blood pressure group[(2.48±0.11)vs(2.36±0.12)vs(2.41±0.26)mmol/L,F=9.810,P<0.001].Correlation analysis showed that systolic and diastolic blood pressure were both positively correlated with blood sodium(r_(s)=0.139,P=0.019 r_(s)=0.140,P=0.018)and blood calcium(r_(s)=0.377,P<0.001,r_(s)=0.301,P<0.001)while negatively correlated with blood sodium to calcium ratio(r=-0.354,-0.264,both P<0.001).Multiple stepwise linear regression analysis showed that both systolic and diastolic blood pressure was positively correlated with BMI(β=0.304,P=0.028 and β=0.130,P=0.032),blood sodium(β=0.121,P=0.013,β=0.124,P=0.021)and blood calcium(β=0.336,0.273,both P<0.001).Conclusions College students with hypertension have higher blood calcium than non-hypertension students.Blood calcium is correlated with systolic and diastolic blood pressure.
