摘  要:目的探讨成人脂质蓄积指数(LAP)与高血压、糖尿病的关系。方法纳入前瞻性城乡流行病学研究中国队列(PURE-China)的社区人群共37233人。参照公式LAP(男)=[腰围(cm)-61.3]×三酰甘油(mmol/L),LAP(女)=[腰围(cm)-55.6]×三酰甘油(mmol/L)计算LAP。依据LAP(cm·mmol/L)三分位数和体质量指数(BMI,kg/m^(2))分组[男性LAP Q1(<14.88)、Q2(14.88~50.77)、Q3(>50.77),女性LAP Q1(<16.28)、Q2(16.28~49.14)、Q3(>49.14);非超重组(BMI<24.0)、超重组(24.0~<28.0)、肥胖组(≥28.0)],采用广义线性混合效应模型分析男性和女性LAP与高血压、糖尿病的关系。结果男性和女性人群高血压、糖尿病患病风险均随着LAP水平升高而增加。与LAP Q1组相比,女性和男性LAP Q3组高血压患病风险OR(95%CI)值分别为2.291(1.935~2.714)、2.381(1.998~2.837),糖尿病患病风险分别为1.555(1.301~1.859)、1.716(1.424~2.069)。与LAP Q1+非超重组相比,LAP Q3+非超重组女性高血压和糖尿病的患病风险OR(95%CI)值分别为3.353(2.932~3.835)和5.946(4.726~7.481),男性分别为1.988(1.690~2.338)和4.637(3.538~6.077);LAP Q1+肥胖组女性高血压和糖尿病的患病风险OR(95%CI)值分别为1.973(1.414~2.754)和0.453(0.111~1.848),男性分别为2.231(1.365~3.644)和1.998(0.715~5.580)。结论LAP与高血压及糖尿病患病风险密切相关。Objective To investigate the relationship between lipid accumulation product(LAP)and hypertension,diabetes in adults.Methods A total of 37233 community residents from Chinese prospective urban and rural epidemiological study(PURE-China)were included.LAP was established as the following formula:LAP(male)=(waist circumference-61.3)×triacylglycerol,and LAP(female)=(waist circumference-55.6)×triacylglycerol.The participants were grouped according to the LAP(cm·mmol/L)tertiles and body mass index(kg/m^(2))[male:LAP Q1(<14.88),Q2(14.88-50.77),Q3(>50.77),female:LAP Q1(<16.28),Q2(16.28-49.14),Q3(>49.14);non-overweight(BMI<24.0),overweight(24.0-<28.0),obesity(≥28.0)].The relationship between LAP and hypertension,diabetes in different genders was analyzed by generalized linear mixed model.Results The risk of hypertension and diabetes increased with the increase of LAP both in male and female.Compared with LAP Q1 group,the OR(95%CI)for hypertension in LAP Q3 group was 2.291(1.935-2.714)in female and 2.381(1.998-2.837)in male respectively.The corresponding OR for diabetes was 1.555(1.301-1.859)in female and 1.716(1.424-2.069)in male,respectively.Compared with LAP Q1 and non-overweight group,the OR(95%CI)for hypertension and diabetes in LAP Q3 and non-overweight group were 3.353(2.932-3.835),5.946(4.726-7.481)in female,and 1.988(1.690-2.338),4.637(3.538-6.077)in male,respectively.Compared with LAP Q1 and obesity group,the OR(95%CI)for hypertension and diabetes in LAP Q1 and obesity group were 1.973(1.414-2.754),0.453(0.111-1.848)in female,and 2.231(1.365-3.644),1.998(0.715-5.580)in male,respectively.Conclusion Increased LAP is significantly associated with a high risk of hypertension and diabetes.
