摘　　要：目的探讨≥65岁老年人正常范围内血清丙氨酸氨基转移酶(alanine aminotransferase,ALT)与代谢综合征(metabolic syndrome,MS)及其组分的相关性和剂量-反应关系。方法以2019年常州市武进区≥65岁健康体检人群为研究对象,采用logistic回归和限制性立方样条分析ALT与MS患病的相关性和剂量-反应关系。结果共91889人纳入研究,MS患者30555人,占33.25%。随着血清ALT水平的增加,MS及其各组分的患病率逐渐增加(χ^(2)值为45.47~1177.10,P值均<0.05)。调整相关混杂因素后,男性:与ALT<13.9 U/L组相比,ALT在13.9~17.9、18.0~23.4、23.5~40.0 U/L组,患MS的OR(95%CI)分别为1.39(1.30~1.48)、1.83(1.72~1.95)、2.74(2.57~2.92);女性:与ALT<13.0 U/L组相比,ALT在13.0~16.9、17.0~21.9、22.0~40.0U/L组,患MS的OR(95%CI)分别为1.28(1.21~1.35)、1.51(1.43~1.60)和2.33(2.21~2.46)。限制性立方样条分析显示,调整相关混杂因素后,男、女血清ALT水平与MS患病率均呈线性剂量-反应关系(非线性检验P值分别为0.77、0.46)。结论≥65岁老年人正常范围内血清ALT水平与MS及其组分相关,且呈线性剂量-反应关系。Objective To investigate the correlation and dose-response relationship between serum alanine aminotransferase(ALT)within the normal range and metabolic syndrome(MS)and its components in elderly population aged at least 65 years old.Methods The elderly population aged at least 65 years old taking medical examination in Wujin District of Changzhou City participants in 2019 were chosen as subjects.The logistic regression and restricted cubic spline method were used to analyze the correlation and dose-response relationship between serum ALT and MS.Results A total of 91889 participants were included in the study,including 30555 MS patients,accounted for 33.25%.With the increase of ALT level,the prevalence rates of MS and its components increased gradually(χ^(2) values were from 45.47 to 1177.10,all P<0.05).After adjusting for confounding factors,the OR(95%CI)of MS in ATL level of(13.9-17.9),(18.0-23.4)and(23.5-40.0)U/L groups in males were 1.39(1.30-1.48),1.83(1.72-1.95)and 2.74(2.57-2.92)compared with the ALT level of<13.9 U/L group;and OR(95%CI)of MS in ATL level of(13.0-16.9),(17.0-21.9)and(22.0-40.0)U/L group in females were 1.28(1.21-1.35),1.51(1.43-1.60)and 2.33(2.21-2.46)compared with the ALT level of<13.0 U/L group.The results of restricted cubic spline analysis showed linear dose-response relationships between serum ALT levels and MS in both genders(nonlinear test P values were 0.77 and 0.46,restpetively).Conclusion Serum ALT level was correlated with MS and its components in elderly population aged at least 65 years old,demonstrating linear dose-response relationship.
