加味四黄汤内服外敷对老年混合痔术后康复及炎性因子的影响

摘　　要：目的探讨加味四黄汤内服外敷对老年混合痔患者术后康复及炎性因子的影响。方法选择沈阳市肛肠医院接收的老年混合痔患者69例作为研究对象,收治入院时间为2018年4月—2020年4月,所有患者统一接受吻合器痔上黏膜环形切除术治疗,根据其术后不同处理方法进行分组,将采用高锰酸钾溶液坐浴治疗的患者纳入参照组(35例),将采用加味四黄汤内服外敷治疗的患者纳入研究组(34例)。观察和比较2组患者疼痛消失时间、创面愈合时间、创面渗液消失时间、水肿消失时间和治疗前后白细胞介素-6、γ-干扰素、血清血管内皮生长因子的变化。结果与参照组相比,研究组疼痛消失时间、创面愈合时间、创面渗液消失时间、水肿消失时间较短(P<0.05)。治疗前,2组患者白细胞介素-6、γ-干扰素、血清血管内皮生长因子相比无显著差异(P>0.05);治疗后,2组患者白细胞介素-6水平显著降低、血清血管内皮生长因子、γ-干扰素水平显著提升,且与参照组相比,研究组白细胞介素-6较低,γ-干扰素、血清血管内皮生长因子水平较高,差异均有统计学意义(P<0.05)。结论在老年混合痔患者手术治疗后给予加味四黄汤内服外敷,不仅能促进患者术后康复,还能显著缓解其炎症。Objective To investigate the effect of internal and external application of modified Sihuang decoction on postoperative rehabilitation and inflammatory factors in elderly patients with mixed hemorrhoids.Methods 69 elderly patients with mixed hemorrhoids received by our hospital were selected as the subjects.The admission time was from April 2018 to April 2020.All patients received circular hemorrhoid mucosal resection with stapler,and were divided into groups according to different postoperative treatment methods.Patients treated with potassium permanganate solution for sit-bath treatment were included in the reference group(35 cases),and patients treated with modified Sihuang Decoction for internal and external application were included in the study group(34 cases).The pain disappeared time,wound healing time,wound exudation disappeared time,edema disappeared time and the changes of interleukin-6,interferon γ and serum vascular endothelial growth factor before and after treatment were observed and compared between 2 groups.Results Compared with the reference group,the time of pain disappearance,wound healing,wound exudate disappearance and edema disappearance were shorter in the study group(P<0.05).There was no significant difference in interleukin-6,γ-interferon and serum vascular endothelial growth factor between the two groups before treatment(P>0.05).After treatment,the levels of interleukin-6 decreased significantly,serum vascular endothelial growth factor andγ-interferon increased significantly The IL-6 levels in the study group were lower than those in the reference group,and the levels ofγ-interferon and serum vascular endothelial growth factor were higher(P<0.05).Conclusion The internal and external application of modified Sihuang decoction can not only promote senile mixed hemorrhoids can not only promote postoperative rehabilitation,but also significantly alleviate inflammation.
