摘 要:目的:探讨O臂导航系统在骶髂螺钉置钉中的应用价值。方法:回顾性分析2015年1月至2019年7月广西贵港市人民医院收住的适合用骶髂螺钉内固定治疗的骨盆损伤患者的临床资料,按照手术中辅助方式分为观察组(20例)和对照组(21例),观察组用O臂导航系统引导行骶髂关节空心螺钉置,对照组采用C臂透视辅助下行骶髂关节空心螺钉置入。比较两组的手术时间、术中透视时间和次数、置钉准确情况和骨折愈合时间等指标。结果:观察组每枚螺钉置入时间、术中透视时间和透视次数均低于对照组,置钉优良率高于对照组(均P<0.05)。两组骨折愈合时间对比,差异无统计学意义(P>0.05)。观察组患者术后未出现神经血管损伤等并发症,而对照组置钉有2例患者术后出现神经根损伤表现。结论:应用O臂导航引导行骶髂螺钉置钉,能缩短手术时间,提高骶髂螺钉置入的精准度,减少术中患者及医务人员X线损害,手术操作较安全,符合精准医疗原则。Objective:To explore the value of O-arm navigation system application in sacroiliac screw placement.Methods:The clinical data of patients with pelvic injury received sacroiliac screw internal fixation in Guigang People's Hospital from January 2015 to July 2019 were retrospectively analyzed.The patients were divided into an observation group(n=20)and control group(n=21)according to auxiliary treatment methods during operation.The patients in the observation group underwent sacroiliac joint cannulated screw placement guided by Oarm navigation system,while those in the control group received C-arm fluoroscopy assisted sacroiliac joint cannulated screw placement.The operation time,fluoroscopy time and times,nail placement accuracy and fracture healing time were compared between the two groups.Results:The screw placement time,fluoroscopy time and fluoroscopy times in the observation group were lower than those in the control group,while the excellent and good rate of screw placement was higher(P<0.05).There was no significant difference in fracture healing time between the two groups(P>0.05).There were no neurovascular injury and other complications after surgery in the observation group and 2 cases of nerve root injury in the control group.Conclusion:Sacroiliac screw placement guided by O-arm navigation can shorten the operation time,improve the accuracy of the internal fixation,and reduce the X-ray damage of patients and medical staff during the operation.The operation is safer and conforms to the principles of precision medical treatment.
