摘  要:目的探讨2021年广州地区聚集性新型冠状病毒Delta变异株感染患者的首次CT表现,为新型冠状病毒变异株肺炎的诊治、病情评估及防控提供参考。方法收集新型冠状病毒Delta变异株感染患者的临床及影像资料进行分析。结果入组78例患者,男28例(35.9%),女50例(64.1%),年龄2~85岁,其中16岁以下9例(11.5%),17~40岁18例(23.1%),41~60岁30例(38.5%),60岁以上21例(26.9%),合并基础疾病者26例(33.3%);临床分型轻型23例(29.5%),普通型55例(70.5%),初次评估未见重型及危重型病例。入院首次CT检查阴性者23例(29.5%),CT检查发现病灶者55例(70.5%),多分布在胸膜下,病变形态具有多样性,表现为单纯肺内磨玻璃影35例(44.9%),磨玻璃影合并小叶内间隔增厚(铺路石征)20例(25.6%),磨玻璃影合并纤维条索影19例(24.4%),斑片影伴局灶实变12例(15.4%),小叶中心结节伴晕征10例(12.8%),磨玻璃影伴增粗血管影9例(11.5%)。结论广州地区聚集性新型冠状病毒Delta变异株感染患者大多以主动筛查检测核酸确诊并及早得到相应治疗,首次肺部CT表现以分布在胸膜下的磨玻璃影为主,多合并有小叶间隔增厚。Objective To investigate the initial CT findings of patients with local clustered novel coronavirus mutant strain(Delta) pneumonia in Guangzhou in 2021, and to provide references for the diagnosis, treatment, disease evaluation, prevention and control.Methods The clinical and imaging data of 78 patients with novel coronavirus variant strain(Delta) pneumonia were collected and analyzed.Results Among the 78 patients, there were 28 males(35.9%) and 50 females(64.1%). Among them, the oldest patient was 85 years old, the youngest patient was 2 years old, 9 cases were under 16 years old(11.5%), 18 cases aged between 17-40 years old(23.1%), 30 cases aged 41-60 years old(38.5%), and 21 cases over 60 years old(26.9%). Twenty-six cases were complicated with underlying diseases(33.3%). The clinical classification was mild in 23 cases(29.5%) and common type in 55 cases(70.5%). No severe or critical cases were found in the initial evaluation. The first CT examination was negative in 23 cases, and CT found lesions in 55 cases(70.5%). The initial lesions were mainly subpleural distribution, including simple pulmonary ground glass opacity in 35 cases(44.9%), ground glass opacity associated with interlobular septal thickening(paving-stone pattern) in 20 cases(25.6%), ground glass opacity associated with fibrous strip in 19 cases(24.4%), patch shadow associated with focal consolidation in 12 cases(15.4%), central lobular nodule associated with halo sign in 10 cases(12.8%), and ground glass opacity associated with thickened vascular shadow in 9 cases(11.5%).Conclusions All the patients with pneumonia caused by novel coronavirus mutant stain(Delta) are diagnosed by active screening and detection of nucleic acid and get corresponding treatment as soon as possible. The initial pulmonary CT findings are mainly ground glass opacity distributed under the pleura.
