门急诊抗菌药物处方点评与分析

摘　　要：目的:对本院门急诊抗菌药物进行点评与分析。方法:选取2019年1月-2020年12月应用抗菌药物的门急诊处方1200例,对处方进行点评,并对不合理处方进行分析。结果:1200张处方中,发现不合格处方135张(11.25%)。遴选药品不合宜及无适应证用药的比例最高,与其他不合理用药比例比较,差异有统计学意义(P<0.05)。处方点评中不合理处方分布科室包括急诊科(37.04%),妇科(15.56%),普外科(10.37%),内科(8.89%),眼科(8.15%),皮肤科(7.41%),口腔科(2.96%),儿科(1.48%),其他(7.41%)。门急诊抗菌药物不合理应用种类最多的为β-内酰胺类抗生素(70%),其次为大环内酯类抗生素(10%)。结论:本院门急诊应用抗菌药物存在一些问题,应在开展处方点评的基础上进一步加强抗菌药物合理应用管理。Objective:To review and analyze the antibacterial drugs in the outpatient and emergency departments of our hospital.Methods:From January 2019 to December 2020,1200 outpatient and emergency prescriptions using antibacterial drugs were selected,the prescriptions were reviewed,and the unreasonable prescriptions were analyzed.Results:Among the 1200 prescriptions,135 unqualified prescriptions were found(11.25%).The proportion of inappropriate selection of drugs and medication without indication was the highest,and there was significant difference compared with other unreasonable medications(P<0.05).The unreasonable prescription distribution departments in the prescription reviews include emergency department(37.04%),gynecology(15.56%),general surgery(10.37%),internal medicine(8.89%),ophthalmology(8.15%),dermatology(7.41%),stomatology(2.96%),pediatrics(1.48%),others(7.41%).The most kinds of irrational use of antibiotics in outpatient and emergency departments wereβ-Lactam antibiotics(70%),followed by macrolide antibiotics(10%).Conclusion:There are some problems in the application of antibacterial drugs in the outpatient and emergency departments of this hospital.The rational application management of antibacterial drugs should be further strengthened on the basis of the prescription review.
