摘　　要：目的探讨在腹腔镜下行低位直肠癌手术过程中,使用自制精准除烟设备吸除使用电外科设备时产生的烟雾的效果。方法采用类实验法,在长沙某三甲医院选取80例行腹腔镜下低位直肠癌手术的患者为研究对象,采用随机数字表法将其分为两组,每组各40例。实验组采用自制精准除烟设备排烟,对照组使用传统方法排烟。对比两组患者手术时长、术中因视野不清暂停次数、擦镜次数、手术出血量及术者的满意度、对术中术野清晰度评分。结果实验组的手术时长、暂停次数、擦镜次数、出血量少于对照组,且满意度、术野清晰度评分均高于对照组,差异有统计学意义(P<0.05)。结论自制的精准除烟设备可以在保持术野清晰、腹腔内压力恒定的情况下精准有效地吸除手术过程中使用电外科设备产生的烟雾,且不影响主刀医生手术操作,既可使手术安全顺利地进行,又可保证手术室的环境质量不被污染,保护手术室人员的身体健康。Objective To investigate the effect of a self-made precise smoke evacuation equipment to prevent the surgical smoke generated by electrosurgical system during laparoscopic surgery for patients with low rectal cancer.Methods Eighty patients undergoing laparoscopic surgery for low rectal cancer in a thirdgrade A hospital were selected as the research subjects by a similar experimental method.They were divided into the experimental group and the control group according to the random number method,with 40 subjects in each.The experimental group used self-made precise smoke evacuation equipment,while the control group used traditional methods to exhaust smoke.The duration of operation,the times of operation pauses caused by unclear visual field,the lens erasing times,the intra-operative blood loss,the satisfaction score of the operators,and the evaluation on the clarity of the operative field were compared between the two groups.Results The duration of operation,the times of operation suspension,the times of lens erasing and the intra-operative blood loss of the experimental group were lower than that of the control group,and the scores of operators'satisfaction and clarity of the operative field were higher than that of the control group with statistically significant(P<0.05).Conclusions The self-made precise smoke evacuation equipment can remove the smoke generated by the electro-surgical equipment during the operation accurately and effectively as well as keep the operating field clear and the intra-abdominal pressure constant,without affecting the surgical process of the surgeons.It can not only make the operation safe and smooth,but also ensure the non-pollution of the environment of the operating room and protect the health of the staff.
