NPWT联合银离子敷料治疗糖尿病足部溃汤的疗效分析

摘　　要：目的探讨负压创面治疗(NPWT)联合银离子敷料治疗糖尿病足部溃疡的临床效果。方法选取2018年1月至2020年1月在我科手术治疗的18例Wagner分级为2~4级的糖尿病足部溃疡患者,创面清创后采用NPWT联合银离子敷料治疗,2个治疗周期后观察创面肉芽组织覆盖率、肉芽组织生长厚度、细菌清除率及愈合时间。结果所有患者足部创面在治疗2个周期后可见到大量肉芽组织生长,创面肉芽组织覆盖率为32.56%±11.52%,创面肉芽组织生长厚度为(3.26±0.84)mm,细菌清除率达82.56%±12.48%。12例患者行皮片移植后创面愈合,愈合时间平均(22.2±1.9)d;6例患者清创后直接缝合,平均愈合时间为(17.4±2.2)d。结论NPWT联合银离子敷料可有效促进肉芽组织生长,减少细菌感染,加速创面愈合,效果确切,值得临床推广。Objective To explore the clinical effect of negative pressure wound therapy(NPWT)combined with silver ionic dressing in the treatment of diabetic foot ulcers.Methods From January 2018 to January 2020,a total of 18 patients suffered diabetic foot ulcers with Wagner grades 2 to 4 treated in our department were selected and analyzed retrospectively.After debridement of the wound,NPWT(negative pressure suction)combined with silver ionic dressing was used for two treatment periods.The coverage rate of wound granulation tissue,the growth thickness of wound granulation tissue,the clearance rate of wound bacteria and the time of wound healing were observed.Results A large amount of granulation tissue grew on the wounds in all patients after 2 periods of treatment.The granulation tissue coverage rate of the wound was 32.56%±11.52%,the thickness of the wound granulation tissue growth was(3.26±0.84)mm,and the bacterial clearance rate was 82.56%±12.48%.The wounds healed in 12 cases after skin grafting with the average healing time of(22.2±1.9)days,and the wounds were sutured directly after debridement in 6 patients with the average healing time of(17.4±2.2)days.Conclusion The NPWT combined with silver ionic dressing can effectively promote the growth of granulation tissue,reduce bacterial infection and accelerate wound healing,and is worthy of clinical promotion.
